A Prospective Analyze Of Adolescents Contraceptive Choice In Our Clinic
Kontrasepsiyon İstemi İle Kliniğimize Başvuran Adölesanların Değerlendirilmesi

ÖZET
Amaç: 1995-2000 yılları arasında polikliniğimize kontrasepsiyon istemiyle başvuran adolesan olguların tercih ettikleri yöntem ve bu yöntemden memnuniyetlerinin prospektif olarak değerlendirilmesi.Materyal ve Metod: 1995-2000 yılları arasında kontrasepsiyon istemiyle kliniğimize başvuran 14-18 yaş arası 320 adolesan olgu çalışmaya dahil edildi. Hastaların yaşları, gebelik sayıları, medeni durumları ve tercih ettikleri kontraseptif yöntemler değerlendirildi. Hastalar kontraseptif uygulamadan 6 ay sonra kontrole çağrıldı ve kullandıkları kontraseptif yöntemden memnuniyetleri sorgulandı.Bulgular: Çalışmaya alınan 320 olgunun yaş ortalaması 17,3±09 idi. 178 (%55,7) inin sadece bir gebeliği, 112 (%35) olgunun gebelik öyküsü bulunmazken, 30 (%9,3)ünün ise birden fazla gebelik öyküsü mevcut idi. 320 olgunun 47 (%14,6)si bekar, 65 (%20,3)inin evlilik süresi bir yıldan az, 208 olgunun ise evlilik süreleri bir yıldan fazla idi. Kontraseptif yöntem olarak aylık doğum kontrol haplarını kullanan 172 (%53,7) olgunun 137 (%80)si kendi isteğiyle bu yöntemi terch etmiş idi. 35 olgu ise polikliniğimizde verilen bilgilendirmeden sonra kendi isteği ile bu yöntemi tercih etti. Doğum kontrol hapı kullanan 172 olgunun 133 (%77,3)ü 6 aylık kontrollerinde, bu yöntemden memnuniyetini belirtirken, kalan 37 (%22,7) olgu ise çeşitli nedenlerle ilacı kullanmayı bırakmış idi. Rahim içi araç (RIA) kullanan 66 (%20,7) olgunun %90nı kendi isteği ile bu yöntemi tercih etmiş idi. Kalan %10?nu bilgilendirme sonrası tercih etti. 56 (%84) olgu kontrolleri sırasında bu yöntemden memnuniyetini bildirirken, 10 (%16) olgu ise adet kanamalarının sıklığı ve miktarında artma, kasık ve bel ağrısı nedeniyle bu yöntemi kullanmak istemediğini bildirdi. Kalan 82 (%25,6) olgu ise, kontraseptif yöntem olarak kondom kullanmayı tercih etmiş idi. Bu olguların tamamı kontrollerinde bu yöntemi kullanmaya devam edeceklerini bildirdi. Sonuç: Adölesanlarda doğum kontrol hapları en sık tercih edilen yöntem olarak bildirilmektedir. İkinci sıklıkta kondom daha sonra RIA kullanılmaktadır. Kondom batı toplumlarına kıyasla, bizim toplumumuzda daha az sıklıkta kullanılmaktadır. Bu durum cinsel yolla bulaşan hastalıklar açısından önem taşımaktadır.
ABSTRACT
Aim: To analyze the contraception method choice and satisfaction in adolescents who attempted to our clinic between 1995-2000 years.Material and Methods: In this study 320 adolescents between 14-18 years age who attempted to our clinic with contraception request were enrolled. Age, parity, gravidy, abortion, marital status and contraception method choice were investigated. The satisfactory of the patients with contraceptive methods were studied after six months from initiation of contraception.Results: The mean age was 17.3±0.9. There were no pregnancies in 112 (%35) patients, only one pregnancy in 178 (%55.7) patients and one or more pregnancies in 30 (%9.3) patients history. Forty seven (%14.6) patients were single, 65 (%20.3) patients were married a duration less than a year and 208 patients were married a duration more than a year.The contraceptive method choice was oral contraceptive pills in 172 (%53.7) of patients and in 137 (%80) of these cases the contraceptive choice was their own decision. In the remaining 35 cases oral contraceptives were suggested from our clinic. While the satisfactory was positive in 133 (%80) cases of 172 oral contraceptive used patients in six months follow-up, the rest of the cases (35) were quit contraception because of different reasons. Ninety percent of 66 (%20.7) intrauterine device user patients made their own decision. The rest of the IUD users made their own decision. In these patients 56 (%84) of the cases were satisfactored and 10 (%16) cases were quit because of irregular menses, altered menstrual bleeding, inguinal and abdominal pain. In the rest 82 (%25.6) cases, condoms were selected as contraceptive method. In follow-up all of these cases were comfortable with this method. Conclusion: Oral contraceptives were reported as the most common choice in adolescents for contraception. The other common contraception choices in descending order of adolescents were intrauterine device and condoms. In our country the condom employ was less than civilized countries, this method plays an important role in sexual transmitted diseases incidence.
