ÖZET
Mikrokimerizm, bireyin kendisine ait olmayan, başka bir bireyden kaynaklanan yabancı hücrelerin veya DNA parçalarının bireyin dokularında veya dolaşımındaki varlığıdır. Mikrokimerzmin en sık nedeni gebelik olmakla birlikte kan transfüzyonu ve organ transplantasyonu da nedenler arasında yer almaktadır. Fetal DNA veya hücrenin maternal dolaşımda varlığı feto-maternal mikrokimerizm olarak tanımlanır. Fetal hücre veya DNA'nın maternal dolaşıma geçişinin, erken gebelik haftalarında başladığı ve artan gestasyonel yaş ile maternal dolaşımdaki varlığının arttığı belirtilmiştir. Fetal mikrokimerik hücreler en erken gebeliğin 4-5. haftalarında maternal kanda tespit edilmiştir. Feto-maternal mikrokimerizm elektif gebelik terminasyonundan ve tekrarlayan gebelik kayıplarından sonra da rapor edilmiştir. Mikrokimerik fetal hücrelerin birçok organ ve dokuda kolonize olabileceği düşünülmektedir. Fetüste az miktarda bulunan maternal hücre veya DNA materno-fetal mikrokimerizm olarak bilinir. Fetüste, anneye ait eritrositlerin ve lökositlerin varlığını ve konjenital anomalili erkek bebeklerin periferik kanında XX karyotipli hücreleri tespit eden çeşitli çalışmalar vardır. Gebelik sırasında bu iki yönlü kimerik hücre geçişi sistemik skleroz, primer bilier siroz, Sjögren sendromu, sistemik lupus eritematozus, Tip 1 diyabet, tiroidit ve romatoid artrit gibi bazı otoimmün hastalıkların ve gebeliğin polimorfik erüpsiyonu, preeklampsi, kanserler, hepatit C, tiroid adenomları, multinodüler guatr gibi otoimmün olmayan hastalıkların patogenezinde rol oynayabilir. Mikrokimerizm, hastalıkların patogenezini açıklamaya katkı sağlayacak ve tedavisinde umut vadedecek, noninvaziv prenatal tanı yöntemi olarak da kullanılabilecek, aydınlatılmayı bekleyen güncel bir konudur.
ABSTRACT
Microchimerism is the presence of a small amount of foreign cells or DNA within the tissues or circulation of an individual. The most common source of microchimerism is pregnancy. Microchimerism can also occur due to blood transfusions, organ transplants. The presence of low numbers of fetal cells or DNA in mother is known as fetal-maternal microchimerism. The presence of low numbers of fetal cells or DNA in mother is known as fetal-maternal microchimerism. Fetal microchimeric cells (MC) is detectable in maternal blood as early as 4-5 weeks gestational age. However, the frequency of these cells in fetal circulation declines with advancing gestational age. Fetal-maternal microchimerism has been reported after electively terminated pregnancies and patients with recurrent spontaneous abortions. Microchimeric fetal cells are thought to have the ability to colonize multiple tissues and organs. The presence of low numbers of maternal cells or DNA in offspring is known as maternal-fetal microchimerism. There are several reports about the detection of maternal erythrocytes and leukocytes in the fetus, and cells carrying the XX karyotype have been detected in the peripheral blood of a male baby with congenital abnormalities These observations, together with the bi-directional trafficking of cells during pregnancy, have suggested the possible role of fetal-maternal or maternal-fetal microchimerism in the pathogenesis of some autoimmune diseases such as systemic sclerosis, primary biliary cirrhosis, Sjögren syndrome, systemic lupus erythematosus, type 1 diabetes mellitus, thyroiditis and rheumatoid arthritis. Also the role of microchimeric cells in the pathogenesis of non-autoimmune diseases such as polymorphic eruptions of pregnancy, pre¬eclampsia, cancers, hepatitis C, thyroid adenoma and multinodular goitre, is still controversial. Future studies will have to address the identification of infiltrating microchimeric cells, the functions of these cells, and the consequences of treatments of autoimmune disease on microchimerism, before fully determining the role of this phenomenon in human diseases and it may be used as a method of non-invasive prenatal diagnosis in the future.
