Clinical Experience With Tibolone (Livial) Over 3 Years

ÖZET
Amaç : Bir sentetik steroid olan tiboloneun 3 yıllık kullanım süresinde ve sonunda yan etkilerini ve postmenapozal hastaya katkılarını belirlemek. Çalışmanın yapıldığı yer: Dr. Zekai Tahir Burak Kadın Hastanesi Menopoz Bölümü. Materyal ve metod: Çalışma1994-1997 yılları arasında çeşitli sebeplerle tibolone kullanılması uygun bulunan yaş ortalamaları 55.8 olan 600 postmenopozal hasta üzerinde yapıldı. İlaç başlanması planlanan hastaların rutin biyokimyasal tetkikleri ve eğer son 1 yıl içinde yapılmamışsa mamografileri , transvajinal endometrial kalınlık ölçümleri yapıldı. Eğer kullanmışlarsa daha önceki tedavi sırasındaki yan etkiler ve şu andaki klimakterik şikayetleri öğrenildi. Tibolon dozu 2,5 mg/gün idi. Hastalar ilk olarak 6 hafta sonra daha sonra ise 6 ayda bir kontrole çağrıldılar. Kontrollerde anamnez alınıp transvaginal endometrial kalınlık ölçümleri ve meme muayeneleri yapıldı . Bulgular: Tedaviye devam eden 459 hastada tibolon iyi tolere edilmiş ve klimakterik semptomlar 3-5 hafta içinde düzelmiştir. En önemli yan etki kilo alma ve kadınların 56sında (%12.38) görülen ödem ve şişkinliğe eğilim olmuştur. Vajinal kanama 57 kadında (%12.4) görülmüş olup bunlardan 25 tanesindeki kanamanın daha önce aldıkları östrojen tedavisine bağlı olduğuna karar verilmiştir. Yirmidört kadında kanama nedeni olarak polip veya fibroid tesbit edilmiş, kalan sekizinde ise herhangi bir neden bulunamamıştır. Meme semptomları sadece %7.52 oranında bildirilmiş ve benign meme hastalığı sebebiyle gönderilen 27 hastanın hiçbirinde memeyle ilgili bir rahatsızlık görülmemiştir. Yan etkiler nedeniyle ilacı bırakma oranı % 3dür(14 hasta).Sonuç: Menopozal tedavideki kararlar maalesef sadece bilimsel sonuçlara dayanarak verilmemektedir. Hastalar bu konudaki bilgileri doktorlardan çok medyadan ve arkadaşlarından almaktadırlar. Bir başka yanlışda doktor ve hasta açısından en çok rahatsız eden semptomlardan kurtulmanın yeterli sayılıp iskelet ve ürogenital sistemin uzun vadeli korunmasının gözardı edilmesidir. Kanamasız hastalar için tibolon ile tedaviye uyum daha iyidir. Siklik estrojen ve progesteron tedavisinde olduğu gibi kanama olunca hastalar tedaviyi kolaylıkla reddedebilmektedir.
ABSTRACT
Objective : The aim of the study was to assess side effects during and after 3 years use of tibolone,a synthetic steroid. Institution : Dr. Zekai Tahir Burak Womans Hospital Material and method : The study was undertaken between years 1994 and 1997 among 600 postmenopausal women who were found to be suitable for tibolone use for various reasons. Mean age was 55,8 years. All were analysed for routine biochemical parameters . All but ones who had mammograpy and endometrial thickness measurement during last one year were both sent to mammography and to transvaginal endometrial thickness measurement. The symptoms they were suffering and the side effects during the previous treatment regimen were both asked. Dose of tibolone was 2,5 mg/day . The first visit was planned after 6 weeks of treatment . After the first visit the patients were seen once 6 months . At each visit, symptoms and side effects were asked and both transvaginal endometrial thickness measurement and breast examination performed. Findings: Tibolone was well tolerated by 459 women who adhered to the treatment and climacteric symptoms were resolved in 3-5 weeks. The most evident side effects observed were weight gain and oedema which were encountered in 56 women (12,38 percent). Vaginal bleeding was seen in 57 women (12,4 percent) but in 25 of them the bleeding was decided to be due to the previous oestrogen treatment . In 24 women fibroids or polyps were found as the cause of the bleeding. In the remaining eight no organic cause of the bleeding could be found. Breast symptoms were reported only 7,52 percent and interestingly, in the 27 women who especially referred to the tibolone treatment because of benign breast disease, no breast discomfort observed. The rate of abandonment due to the side effects was only 3 percent (14 women). Conclusion: Decisions on menopausal therapy are unfortunately not always taken on strictly scientific grounds. Women may infact obtain much of their information on the effects of menopausal therapy from the media and from discussion with their friends, rather than fromphysicans. There is a view both in women and doctor that treatment of the menopause is generally a question of short-term relief of symptoms. Long-term consequences such as skeletal protection and maintenance of urogenital integrity on a preventive basis are frequently not considered. That compliance may be better with tibolene since women are more likely to continue therapy if they do not have to bleed, as they do with cyclical oestrogen and progestogens, may therefore be ignored.
