ÖZET
Amaç: Bu çalışmanın amacı gestasyonel diabetes mellitus (GDM)lu hastalarda gelişen antenatal ve peripartum fetomaternal ve neonatal komplikasyonların insidansını saptamak ve bunları pregestasyonel diabetes mellitus (pre-GDM)lu ve diabetik olmayan gebe populasyonun verileri ile karşılaştırmak.Gereç ve Yöntem: Bu çalışmada Ocak 1998 ve Aralık 2000 tarihleri arasında Zekai Tahir Burak Kadın Sağlığı Eğitim ve Araştırma Hastanesi yüksek riskli gebelikler kliniğinde takip edilen GDMli 183, pre-GDMli 39 hasta ve kontrol grubu olarak da diabetik olmayan 164 hasta çalışma kapsamına alınmıştır. Antenatal ve peripartum fetomaternal ve neonatal komplikasyonlara ait veriler bu gruplar arasında karşılaştırılmıştır. Bulgular: Ortalama anne yaşı ve gravidite hem GDM hem de pre-GDM grubunda kontrol grubuna göre yüksek bulunmuştur (p=0.0001). Ayrıca sezaryen oranı, obstetrik ve puerperal komplikasyon oranları da GDM ve pre-GDMli grupta diabetik olmayan gruba göre yüksek bulunmuştur (p<0.05). Hipoglisemi ve hiperbilurubinemi tespit edilen yenidoğan sayısı GDM ve pre-GDMli grupta kontrol grubundakilere göre fazladır (p<0.001, p<0.005). GDMli ve pre-GDMli gruptaki annelerin yenidoğanlarının 5.dakika Apgar skorları ile normoglisemik annelerin yenidoğanlarının 5.dakika Apgar skorları arasında istatistiksel olarak anlamlı bir fark saptanmadı (p> 0.5).Sonuç: Preeklampsi ve preterm eylem gibi obstetrik komplikasyonlar; hipoglisemi, hiperbilüribinemi ve fetal makrozomi gibi neonatal komplikasyonlar gebeliği diabetin tüm formları ile komplike olan anne ve yenidoğanlarında daha sık görülmektedir. Bu nedenle GDMli gebelerin teşhis ve tedavisine Pre-GDMli gebelerinki kadar önem verilmelidir. Böylece erken tanı ve düzenli bir takip ile bu fetomaternal ve neonatal komplikasyonların en aza indirilmesi mümkün olabilir.
ABSTRACT
Objective: The aim of this study is to determine the incidence of antenatal and peripartum fetomaternal and neonatal complications in patients with gestational diabetes mellitus (GDM) and to compare these results with those belonging to patients with pregestational diabetes mellitus (pre-GDM) and non-diabetic pregnant population.Materials and Method: In this study, 183 patients with GDM and 39 patients with pre-GDM, whose follow up were made in high risk pregnancy clinic of Zekai Tahir Burak Woman Health Education and Research Hospital between January 1998 and December 2000, were studied. Also 164 non-diabetic pregnant women were studied as control group. A comparison about antenatal and peripartum, fetomaternal and neonatal complications was made between these groups.Results: The average maternal age and gravidity were found to be higher in both GDM and pre-GDM groups than in control group(p=0.0001). Also the ratio of cesarian section , obstetric and puerperial complications were higher in GDM and pre-GDM patients than in non-diabetic control group(p<0.05). The number of neonates who had hypoglycemia and hyperbilurubinemia were higher in GDM and pre-GDM groups than in control group(p<0.001, p<0.005). There was no statistically significant difference between 5.minute Apgar scores of neonates born to mathers with GDM, pre-GDM and normoglycemia(p>0.05).Conclusion: Obstetric complications like preeclampsia and preterm labor, neonatal complications like hypoglycemia, hyperbilurubinemia and macrosomia are seen more commonly in pregnant women, whose pregnancies are complicated with all forms of diabetes, and in their neonates. Thats why it is equally important to determine and manage pregnant women with both pre-GDM and GDM. Thus it can be possible to minimize these fetomaternal and neonatal complications by an early diagnosis and proper follow up.
