Comparison Of Frozen And Parafin Section Diagnosis Of Ovarian Tumors That Were Reported As Borderline At Frozen Section

ÖZET
Amaç: Over kisti nedeni ile opere olan ve operasyon anında istenen Frozen section incelemesi borderline olarak gelen over tümörlerinde, kalıcı patolojik inceleme sonuçlarının değerlendirilmesi. Gereç ve Yöntemler: 2002 ve 2004 yılları arasında, hastanemizde, frozen section inceleme sonucunda borderline over tümörü teşhisi konulan 52 adet hasta dosyası, retrospektif olarak gözden geçirilmiştir. Hastanın yaşı, tümör büyüklüğü, bilateral ya da ekstraoveryan hastalık varlığı ve birlikte asit tanısı bulunmasına göre sınıflandırılmıştır. Kalıcı patoloji sonuçları ile arasındaki korelasyon değerlendirilmiştir. Bulgular: Çalışmaya dahil edilen hastaların ortalama yaşı 42.8 idi. Hastaların, 20 tanesi seröz, 22 tanesi müsinöz, 6 tanesi endometrioid, 2 tanesi clearcell, 1'er tanesi mikst ve granuloza hücreli olarak tespit edilmiştir. Bütün tümörler değerlendirildiğinde, ortalama büyüklük 14.9 cm (1-30 cm arasında değişmekte), seröz tümörlerde 11.6 cm, müsinöz tümörlerde 18.5 cm.dir. Olguların %50'sinde frozen ile kalıcı patoloji birbiri ile uyumludur. Olguların %30.7'sinde frozene göre borderline lezyonlar malign olarak tanımlanmıştır. Hiçbir değişken yanlış pozitif değerlendirmenin belirteci olmamıştır. Tek değişkenli analizlere göre, yanlış negatif (borderline) değerlendirme, seröz olmayan tümörlerde toplam 32 olguda kalıcı patolojik teşhiste 12'si malign iken, 20 cm'den büyük tümörlerde toplam 10 olgudan 3'ünde kalıcı patolojik teşhis malign olarak gelmiştir. Overe sınırlı olan tümörlerde ise toplam 42 olgudan 11'i kalıcı patolojik teşhiste malign'dir. Sonuç: Bizim çalışmamızda, borderline over tümörleri (BOT)'nin teşhisinde, %50 olguda, frozen ile kalıcı patoloji raporu tutarlılık göstermektedir.
ABSTRACT
Objective: To evaluate the diagnostic accuracy of frozen and parafin section examination of borderline ovarian tumors. Material and Methods: All pathology reports of patients diagnosed to have borderline ovarian tumor at Aegean Maternity Hospital between 2002-2004 were revised. Age of the patients, tumor size, evidence of bilateral or extraovarian diseases and presence of ascite at the time of diagnosis were evaluated. Results: Fifty-two cases were revised. The mean age of the patients was 42.8. In 50% of the cases, frozen section was consitent with parafin section results. In 30.7% of the cases, borderline lesions were more malignant than the frozen sections while 19.2% were reported to be benign. None of the evaluated variables could predict the false positive evaluation. Subgroup analysis was performed for false negative (borderline) diagnosis; with respect to tumor histology 12 of the 32 nonserous tumors were more malignant in the parafin sections, 3 of the 10 cases with a tumor size larger than 20 cm were more malignant while 11 of the 42 cases confined to the ovary were more malignant in the parafin sections. Conclusıon: In our study, the frozen section and parafin section diagnosis of borderline ovarian tumors were consistent in 50% of the cases.
