ÖZET
Epidemiyolojinin orijini, 2000 yıl kadar önce Hipokrat ve diğerlerinin çevresel etmenlerin hastalıklara neden olabileceği fikrinde yatmaktadır. Epidemiyolojide neden-sonuç ilişkisi olarak tarif edilen, aslında, etken ve hastalık arasındaki bağlantı doğru olarak kurgulandığında etkenin ortadan kaldırılması ile hastalığın da ortadan kalkması durumudur. Bir etkene maruziyet sonucu hastalık riskinin artması, hemen her zaman etken ve hastalık arasında nedensel bir ilişki olduğu anlamına gelmez. Etken gibi görünen birçok faktör hastalıkla birlikte görünüp (asosiasyon), hastalığın nedeni olmayan, yani hastalık ile etken arasında bir nedensellik bağlantısı yaratmayan bir konumda yer alabilir. Etken ve hastalık arasında nedensel olmayan ilişki üçüncü bir değişkenin varlığından kaynaklanabilir. 'Karıştırıcı', 'aracı' ve 'etki değiştirici' kavramları, epidemiyolojide sıklıkla tartışılan, obstetri ve jinekoloji alanındaki yayınlarda daha sık rastlanır hâle gelen, nedenselliğe önemli derecede etkisi bulunan üçüncü değişkenlerdir. Bu üç kavram arasındaki fark epidemiyolojinin tartışma konularından biridir ve birbirleri yerine kullanılmaları kavram kargaşasına sebep olmaktadır. Araştırmacılar, ya çalışma dizaynı aşamasında, veriyi toplamadan önce ya da veriyi topladıktan sonra istatistiksel analiz ile bu değişkenleri hesaba katmalıdır. Bu çalışmanın amacı, etken ve sonuç arasındaki nedensel ilişkiyi 'karıştırıcı', 'aracı', 'etki değiştirici' kavramları üzerinden obstetri ve jinekoloji alanından örneklerle tartışmak, bu kavramların nasıl değerlendirilmeleri gerektiği ve çalışma dizaynı ve/veya analiz aşamasında nasıl kontrol edilmeleri gerektiği konusunda literatür ışığında bilgi sunmaktır.
ABSTRACT
Epidemiology was originated from an idea whether the enviromental factors might be the cause of the disease, which was suggested by Hippocrates and the others 2000 years ago. Epidemiology looks for the cause of the disease by which it means that the removal of the risk of disease when the exposure disappear. Increased risk of a health outcome in the presence of an exposure does not necessarily imply a causal relationship between the exposure and the outcome. In many cases, the relationship between the disease and the exposure is a kind of association which means any relationship between two measured quantities. One reason for such non-causal associations is the presence of a third variable called confounder, mediator and effect modifier. These third variables which have serious effect on the relationship between the exposure and outcome are commonly discussed in epidemiology and increasingly mentioned on the papers in obstetrics and gynecology. Although these are conceptually different from each other, it is not always easy to define and distinguish them. The researcher need to account for these variables- either through experimental design and before the data gathering, or through statistical analysis after the data gathering process. The detailed discussion of confounder, mediator and effect modifier is the scope this review. In the light of the literature and with the examples from the field of obstetrics and gynecology, we try to define the methods to select, control and analyse them.
