Crohn's Disease And Pregnancy: A Case Report

ÖZET
Karın ağrısı, bulantı ve kusma yakınması ile başvuran, 21 yaşında, 22 haftalık bir gebeye Crohn hastalığı tanısı konuldu. Mesalazin ve bir hafta sonra ek olarak metilprednizolon tedavisi başlandı. Hastalığın aktif döneminde olduğu saptanan gebenin takipleri sırasında sepsis, masif rektal kanama ve ileus komplikasyonları gelişti. Barsak rezeksiyonu yapılıp kolostomi açıldı. Postoperatif üçüncü günde intrauterin fetal eksitus olduğu tespit edildi. Operasyon sonrası sekizinci ayda kolostomisi kapatılan hasta, halen sağ ve sağlıklı komplikasyonsuz yaşamaktadır. Gebeliğin varlığı Crohn hastalığının seyrini değiştirmez. Hastalığın gebelik üzerine olumsuz etki yapması aktivitesi ile ilişkilidir. Bu nedenle, hastalığın inaktif olduğu dönemde gebeliğin planlanması abortus, düşük doğum ağırlığı, erken doğum ve ölü doğum gibi komplikasyonları azaltabilir. Tedavide pek çok güvenilir seçenek mevcuttur, cerrahi girişim gerektiğinde operasyon mümkün olduğunca fetus matür olana kadar ertelenmelidir.
ABSTRACT
A 21-year-old woman with a gestation of 22 weeks was admitted to our clinic with the complaints of abdominal pain, nausea and vomitting and subsequently had the diagnosis of Crohn's disease. Initially mesalamine therapy was instituted which was combined with methylprednizolone after a week. She was found to be in the active period of the disease and clinical follow-up of the case was unfortunately complicated with sepsis, massive rectal bleeeding and ileus. Colostomy following intestinal resection was performed. Intrauterine fetal demise was noted on the postoperative third day. Colostomy was closed after eight months of the operation. She is still alive and healthy, without complication. Pregnancy does not alter the clinical course of Crohn's disease. Adverse impacts of the disease on pregnancy outcome appears to be associated with disease activity. Planned conception during the remission phase may lead to decrease in obstetric complications such as miscarriage, low birth weight, preterm labour and intrauterine fetal demise. A number of safe therapeutic approaches are present. If possible, surgical intervention should be delayed until the fetal lung maturation is achieved.
