ÖZET
Amaç: Gebelik ve doğum eylemi ile başvuran değişik yaş diliminden kadınların serumlarında ve bebeklerinin kordon serumlarında Elisa ile HSV-2 IgG ve IgM antikorlarının aranması. Çalışmanın Yapıldığı Yer: Ankara Üniversitesi Tıp Fakültesi Mikrobiyoloji ve Klinik Mikrobiyoloji AD, Ankara. Materyel ve Metod: Çalışmada Etlik SSK Kadın Hastalıkları ve Doğumevi Hastanesi ile Ankara Üniversitesi Tıp Fakültesi, Kadın Hastalıkları ve Doğum Anabilim Dalı'na zamanında doğum eylemi ile başvuran, değişik yaş dilimlerinden, 88 hastanın serumunda ve bebeklerinin kordon serumlarında Elisa yöntemi ile HSV-2 IgG ve HSV IgM araştırılmıştır. Bulgular: Bu annelerde HSV-2 IgG 69/88 (%78.4) ve IgM 5/88 (%5.68) seropozitiflik oranları elde edilmiştir. Kordon serumunda ise %77.3 ve %5.68 oranları saptanmıştır. Yaş dilimleri arasında 21-25 yaş grubunda %81.6 HSV-2 IgG seropozitifliğine ulaşılmıştır (%10.5). Anne HSV-2 IgG-IgM 4 olguda pozitifken, 1 olguda HSV-2 IgG negatif ve HSV IgM pozitif bulunmuştur. Çalışma grubundaki 65 kadında HSV IgG pozitifliği saptanırken, IgM negatifliği elde edilmiş, 18 serumda ise HSV-2 IgG-IgM negatifliğine ulaşılmıştır. HSV-2 IgG 68 kordonda pozitif ve 20 kordonda negatif iken, HSV IgM 5 kordonda pozitif, 83 kordonda negatif olarak değerlendirilmiştir. Bunlardan 4'ünde HSV-2 IgG-IgM birlikte pozitif iken, 1'inde HSV-2 IgG negatif, HSV IgM pozitif olarak değerlendirilmiştir. HSV IgM pozitif 4 olgu HSV reinfeksiyonu ya da reaktivasyonu olarak düşünülürken, 1 olgu anne-bebek primer infeksiyonu şeklinde değerlendirilmiştir. HSV-2 IgG ve IgM negatif 19 olgu ise risk grubunu oluşturmuştur. Sonuç: Toplumda yaygın olan genital HSV infeksiyonları, yenidoğanlarda ciddi komplikasyonlar oluşturabilmektedir. Bu nedenle reaktif veya primer HSV infeksiyonlarının ortaya konulması önem taşımaktadır. HSV-2 IgG ve IgM antikorlarının Elisa ile ölçümü, tanı için yardımcı bilgiler verebilmektedir. Bu çalışmada Elisa ile, HSV serolojik profili, anne-kordon serumu ikilisinde, gözden geçirilmiştir.
ABSTRACT
Objective: Detection of HSV-2 IgG and HSV IgM in the sera of both the umbilical cord of the fetus and their mothers by Elisa. Institution: Ankara University Medical School, Department of Microbiology and Clinical Microbiology. Material and Methods: In the study 88 mother and 88 umbilical cord sera samples were tested by Elisa to detect HSV-2 IgG and HSV IgM. The sera of mothers and umbilical cords were taken from Ankara University Medical School, Department of Obstetrics and Gynecology and SSK Etlik Maternity Hospital, Department of Obstetrics. Results: The seropositivity rate of HSV-2 IgG was 69/88 (78.4%) and HSV IgM was 5/88 (5.68%) in the sera of mothers. In the umbilical cord sera the rates were 77.3% and 5.68% respectively. The highest HSV IgM seropositivity was found in the 21-25 age group (10.5%). While HSV-2 IgG and IgM was positive in four mothers, HSV-2 IgG was negative and HSV-2 IgM was positive in one mother. In the study group, HSV-2 IgG-IgM was negative in 18 women while HSV-2 IgG was positive (but HSV-2 IgM was negative) in 65 mothers. HSV-2 IgG was positive in 68 cord sera (negative in 20 sera), HSV-2 IgM was positive in 5 cord sera but it was negative in 83 sera. In these cord sera, HSV-2 IgG-IgM was positive in 4 cases. HSV-2 IgG was negative but IgM was positive in the other baby. HSV-2 IgM positivity in 4 cases may be reinfection or reactivation, but the other mother-baby had primary infection. 19 cases that had HSV IgG-IgM negative was in the risk group. Conclusion: Genital HSV infection is common among general population and it is important for newborn babies because of its severe complications. For that reason it is important to detect reactive or primary HSV infections. To detect HSV-2 IgG and IgM antibodies by elisa give important results for the diagnosis. In this study, serological profile was tested in 88 mother-umblical cord sera by ELISA.
