Diathermy Loop Excision In The Management Of Cervical Intraepithelial Neoplasia

ÖZET
Amaç: Servikal intraepitelyal neoplazilerin tedavisinde loop eksizyon diyaterminin etkinliğini araştırmak. Çalışmanın Yapıldığı Yer: Gülhane Askeri Tıp Akademisi Haydarpaşa Eğitim Hastanesi Kadın Hastalıkları ve Doğum Servisi, İstanbul. Materyel ve Metod: Jinekoloji polikliniğinde alınan rutin PAP smear sonucu diskaryotik olan veya klinik olarak şüphelenildiği için yapılan kolposkopik servikal biyopsi sonucu servikal intraepitelyal neoplazi olarak rapor edilen, invaziv kanserin ekarte edildiği 51 hastaya lokal anestezi altında poliklinik şartlarında loop eksizyon diyatermi uygulandı. İntraoperatif bulgular, eksizyon materyalin patolojisi ve postoperatif takipteki bulgular irdelendi. Bulgular: Olguların %60.8'inde loop eksizyon materyali tek parça halindeydi. Lokal anestezi, eksizyon işlemi ve hemostaz dahil tedavi süresi %78.4 olguda 5 dakikadan azdı. İntraoperatif kanama %23.5 olguda hiç olmadı, %66.7 olguda hafif dereceydi. Loop eksizyon diyatermi hastalar tarafından oldukça iyi tolere edildi; tedavi süresince %86.3 olguda hiç rahatsızlık hissi olmadı. Loop eksizyon materyalinin histolojik incelenmesinde, eksizyon kenarındaki termal hasar zonunun ortalama kalınlığı 45.8 ± 11.7 mikron olarak bulundu ve histolojik değerlendirmeyi etkilemedi. Histolojik olarak %88.6 olguda eksizyonun tam olduğu saptandı. Meydana gelen postoperatif komplikasyonların hepsi de hafif derecedeydi. Tedaviye bağlı servikal stenoz olmadı. Tedaviden 3 ay sonraki sitolojik, histolojik ve kolposkopik değerlendirmede, başarı oranının %91.4 olduğu saptandı; rezidüel hastalık oranı %8.6 idi. Sonuç: Servikal intraepitelyal neoplazi tedavisinde loop eksizyon diyatermi hızlı, uygulaması kolay, güvenli, ucuz bir yöntemdir; lokal anestezi altında poliklinik şartlarında uygulanabilir, hasta toleransı ve başarı oranı yüksektir.
ABSTRACT
Objective: The purpose of our study was to investigate the efficiency of loop diathermy excision in the management of cervical intraepithelial neoplasia. Institution: Gülhane Military Medical Academy Haydarpaşa Training Hospital, Department of Obstetrics and Gynecology. Material and Methods: The study group consisted of 51 patients with dyscaryotic cervical smears or colposcopically directed punch biopsies from clinically suspected areas consistent with cervical intraepithelial neoplasia. Diathermy loop excision was performed after the absence of invasive disease was revealed with cytologic, histologic and colposcopic examinations. Intraoperative findings, histologic results of loop excision specimens and postoperative follow up findings were evaluated. Results: Single cohesive tissue specimens were obtained in 60.8% of patients treated by diathermy loop excision. The time taken to complete the procedure from infiltration of local anesthetic until achieving hemostasis was less than 5 minutes in 78.4% of cases. Intraoperative bleeding did not occur in 23.5% of patients and mild bleeding occured in 66.7% of patients. Diathermy loop excision was well tolerated by patients; 86.3% of patients had no sensation of discomfort during the operation. On histopathologic examination, the mean thickness of thermal damage at the margins of excision was 45.8 ± 11.7 microns, and histologic interpretation was not affected by thermal artifact. Excision was histologically complete in 88.6% of cases. All of the postoperative complications were mild. Stenosis of the cervical canal due to the treatment was not observed. The success rate of diathermy loop excision, as defined by obsence of cytologic, histologic or colposcopic lesions 3 months after treatment was 91.4%; residual disease was detected in 8.6% of cases. Conclusion: Diathermy loop excision is a reliable, safe, time-saving, well-tolerated, inexpensive and efficient technique which can be accomplished as an out-patient procedure under local anesthesia.
