ÖZET
Amaç: Nadir görülen bir durum olarak dilate kardiyomiyopatili bir hastada miada ulaşan gebelik olgusunun sunumu. Çalışmanın Yapıldığı Yer: İnönü Üniversitesi Tıp Fakültesi, Kadın Hastalıkları ve Doğum AD. Materyal ve Metod: Son adet tarihine göre 32 hafta 2 günlük gebeliği saptanan hasta; son günlerde, artan öksürük, nefes darlığı, terleme ve bacaklarda şişlik şikayetleriyle hastanemiz acil servisine başvurdu. Bulgular: Başlangıç muayenesinde, iki taraflı boyun venlerinde dolgunluk vardı. Göğüs muayenesinde iki taraflı bazal raller duyuldu. Apexte 3/6 dereceden pansistolik üfürümü vardı. Ekokardiyografik incelemede dilate kardiyomiyopati tanısı konuldu. Yoğun medikal tedavi ile rahatlamayan ve fetal distres gelişen olguya kombine spinalepidural anestezi ile acil sezaryen yapıldı. Anne ve bebek bir hafta sonra sorunsuz olarak taburcu edildi. Sonuç: Dilate kardiyomiyopatili hastalarda gebelik hem anne hem de bebek hayatı için oldukça risk taşımaktadır. Bu nedenle bu hastalara kadın doğum kliniği yanı sıra kardiyoloji ve anestezi bölümü ile koordine şekilde yaklaşılmalı ve mümkün olan en kısa sürede gebeliğe son verilmelidir.
ABSTRACT
Objective: The presentation of pregnant case with dilated car-diomyopathy reaching term, a rarely seen condition. Institution: Inonu University Medical Faculty, Department of Obstetrics and Gynecology. Material and Method: The pregnant patient with the gestational age of 32 weeks and 2 days according to her last menstrua-tion date applied to the emergency department with the complaints of cough, shortness of breath, diaphoresis and swelling of the legs, all of which occurs recently. Results: Initial examination revealed bilateral jugular venous distention. Chest examination revealed bilateral basal crackles on auscultation. She had a grade 3/6 pansystolic murmur at the apex. She was diagnosed as dilated cardio-myopathy on echocardiographic examinations. Although she was given intensive medical treatment, her status worsened and cesarean section was performed with combined spinal-epidural anesthesia because of fetal distress. The mother and the baby were discharged from hospital one week after de-livery without any complications. Conclusion: Pregnancy in patients with dilated cardiomyopathy can be extremely hazardous for both mother and the baby. Thats why a multidisciplinary approach including obstetric, cardiology and anesthesiology departments with possible consideration of termination of pregnancy may be required as quick as possible in the management of such patients.
