ÖZET
Amaç: Bu randomize çalışmanın amacı oral veya rektal 600 mgr misoprostol ve standart uygulama olan oksitosin infüzyonu ile meterjinin intramüsküler uygulamasının doğumun üçüncü evresindeki etkinliklerinin karşılaştırmaktır.Çalışmanın yapıldığı yer: SSK Ankara Doğumevi ve Kadın Hastalıkları Eğitim Hastanesi.Materyal ve Metod: Çalışmayı tamamlayan 1182 olgu üç grupta incelendi: 1- oral misoprostol (grup 1), 2- rektal misoprostol (Grup2) ve 3- oksitosin infüzyonu ve intramüsküler metilergometrin verilenler (grup 3). Üçüncü evre aktif olarak yönetildi. Erken kord klemplenmesini takiben kontrollü kord traksiyonu ve uterus masajı uygulandı. Postpartum kanama (³ 500 ml kan kaybı) insidansı ve doğumdan 24 saat sonraki hemoglobin düşüşü ölçüldü.Bulgular: Grup 1 ve grup 2 olgularda postpartum kanama insidansı sırasıyla %9 ve %9.8 ile grup 3den (%3.5) istatistiksel olarak anlamlı oranda yüksekti. Hemoglobin ile hematokritteki oluşan düşüş her üç grupta benzerdi. Ek oksitosik ihtiyacının grup 1 ve grup 2 olgularda, anlamlı olarak grup 3 olgulardan fazla olduğu bulundu.Sonuç: Oral ve rektal misoprostol kullanımı, oksitosin ve metilergometrin kombinasyonu ile kıyaslandığında daha az etkili görünmektedir.
ABSTRACT
Objective: The aim of this study is to compare the effectiveness of oral or rectal administration of 600 mgr misoprostol during the third stage of labour in comparison to the conventional oxytocics in reducing postpartum hemorrhage.Instutition: SSK Maternity and Womens Health Teaching Hospital.Methods: In this controlled trial, 1182 women were randomized to receive; 1- oral misoprostol (Group 1), 2- rectal misoprostol (Group 2), and 3- intravenous infusion of oxytocin 10 IU plus intramuscular administration of methylergometrine 1 ml (Group 3). The third stage was managed actively by early cord clamping and controlled cord traction. The main outcome measures were incidence of postpartum hemorrhage (blood loss ³ 500 ml) and fall in hemoglobine concentration level from prior delivery to 24 hours postpartum.Results: The incidence of postpartum hemorrhage was 9% in group 1, 9.8% in group 2; and 3.5% in group 3 which was significantly lower than the first two groups. There were no significant differences between the three groups with regard to drop in hemoglobine concentrations. Significantly more women needed additional oxytocin in group I and II compared with group 3.Conclusion: Oral or rectal misoprostol is significantly less effective than oxytocin plus methylergometrine for prevention of postpartum hemorrhage.
