ÖZET
Amaç: Sonohisterografide tespit edilen endometrial poliplerin tedavisinde Winter polip forsepsi ve dilatasyon küretaj kullanımının etkinliğinin incelenmesi.Çalışmanın yapıldığı yer: Fırat Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı-Elazığ.Materyal ve Metod: Menometroraji şikayeti ile kliniğimize başvuran, fertilitesini tamamlamış premenopozal veya postmenopozal hastaların yapılan sonohisterografilerinde, uterin kavitede endometrial polip tespit edilen 47 vaka prospektif, tanımlayıcı tipte çalışma programına alındı. Hastalara 800 mcg misoprostol/rektal uygulandıktan dört saat sonra lokal anestezi altında serviks 9nolu hegar bujisine kadar dilate edildi. Endometrial polip lokalizasyonu dikkate alınarak Winter polip forsepsi ile uterın kaviteye girildi. Büyük polipler Winter ile çıkarıldı. Daha sonra iki nolu keskin küretle uterin kavite tümüyle küretlendi. Postküratif 1. gün ve 6. ay sonohisterografi yapılarak uterin kavite tekrar değerlendirildi. İstatistiksel yöntem olarak Wilcoxon Rank test ve Paired t test kullanıldı. p<0.05 anlamlı kabul edildi.Bulgular: Çalışmaya alınan 47 vakanın 43ünde müdahale başarıyla tamamlandı. Tekrarlanan birinci günkü sonohisterografide sadece 2 vakada (%5) bazal sonohisterografideki lezyonun sebat ettiği (geniş tabanlı submüköz myom) tespit edildi ve histeroskopik rezeksiyon önerildi Sonuç: Menometroraji şikayeti olan ve fertilitesini tamamlamış premenopozal veya postmenopozal hastalarda, Sonohisterografi ile tespit edilen endometrial poliplerin tedavisinde Winter polip forsepsi ve D/C kullanımı uygun bir tedavi seçeneği olabilir. Müdahaleden önce 800 mcg rektal misoprostol kullanımı müdahaleyi kolaylaştırmaktadır.

ABSTRACT
Objective: To investigate the efficiency of the use of Winter polyp forceps together with dilatation curettage in the treatment of endometrial polyps identified by sonohysterography.Institution: Department of Obstetrics and Gtnecology, Fırat University School of Medicine-Elazig.Material and Method: Forty seven cases who applied to our clinic complaining about menometrorrhagia and who were found to have endometrial polyp in the uterine cavity by sonohysterography were included in a prospective, descriptive study program. Our patıents were at pre-postmenopausal period and they were not infertile. Four hours after the patients were administered 800 mcg misoprostol/rectal, cervix was dilated as far as the number 9 hegar bougie. Uterine cavity was entered by Winter polyp forceps (taking endometrial polyp localization into consideration), big fragments were taken by twisting. Uterine cavity was fully curetted by number 2 sharp curette. Uterine cavity was re-evaluated by sonohysterography in the post-curative 1st day and 6th month. Wilcoxon Rank and Paired t tests were used for statistical analysis, p<0.05 was regarded significant.Results: The operation was successful in 43 cases of the study. The repeated 1st day sonohysterography revealed that only in 2 cases the lesion in the basal sonohysterography persevered (broad-based submucous myoma) and hysteroscopic resection was proposed.Conclusion: Use of Winter polyp forceps and D/C may be an appropriate alternative in the treatment of endometrial polyps identified by sonohysterography. But, this method should not be performed in the infertile patients. Use of 800 mcg rectal mysoprostol facilitates operation.
