ÖZET
Serum CA-125, bir glikoprotein olup endometriozis dâhil birçok benign ve malign jinekolojik hastalıkta yükselebilmektedir. Endometriozis, endometriyal stromal ve glandüler dokuların endometrium dışında bir alanda yerleşmesi olarak tanımlanmaktadır. Daha önce endometrioma nedeniyle operasyon öyküsü olan 38 yaşında hasta, üç yıldır olan şiddetli dismenore şikâyeti ile kliniğimize başvurdu. Ultrason ve manyetik rezonans görüntülemede sol overde 30 mm'lik endometrioma izlendi, CA-125 ise 455 idi. Hastaya löprolid asetat 3,75 aylık başlandı. Kontrol CA-125 283 geldi ve şikâyetleri kayboldu. Takiplerine üç ay boyunca davam edilen hastanın CA-125 değerleri 92'ye kadar düştü. Sol overdeki endometrioma 20 mm oldu. Bu tip olgularda yüksek CA-125 düzeylerinin evre 3-4 endometriozis ile ilişkili olduğu düşünülmeli ve operasyon sırasında olabilecek komplikasyonlar göz önünde bulundurulmalıdır. Hastalara malignite yönünde muayene ve görüntüleme yöntemleri yapılmalı, malignite bulgusu yoksa ve önceden endometriozis tanısı doğrulanmışsa hemen ikincil bir operasyon yerine medikal tedavi düşünülmelidir.

ABSTRACT
Serum CA-125 is a glycoprotein and increases in many benign and malignant gynecological diseases, including endometriosis. Endometriosis is defined as the settlement of endometrial stromal and glandular tissue in an area outside. The 38 year-old patient who had a history of endometrioma due to the operation has recoursed to of our clinic with complaints of severe dysmenorrhea for 3 years. An endometrioma of 30 mm of the left ovarian has been viewed during Ultrasound and magnetic resonance imaging. CA-125 was the 455 at this stage. Leuprolide acetate 3,75 was started to be given to the patient monthly. Check CA-125 occurred the 283 and the patient's complaints disappeared. Were followed up for 3 months, CA-125 levels dropped to 92. Left ovary endometrioma was 20 mm. At this type of cases, of high-CA-125 levels in patients should be considered to be related to stages 3-4 endometriosis and complications to come into existence during surgery should be taken into account. Examination and imaging studies should be applied to patients in the direction of malignancy; if there is no evidence of malignancy and pre-diagnosis of endometriosis is not confirmed, medical treatment should be considered rather than an immediate secondary operation.
