Enterocutaneous Fistula-A Rare Complication After Anterior Vaginal Wall Sling: Case Report

ÖZET
Stres tipi inkontinans cerrahisinde sentetik materyallerin kullanımı son yıllarda hızla artmaktadır. Dolayısıyla bu materyallere bağlı olarak gelişen komplikasyonlar da artmaktadır. Bant ameliyatı sonrası komplikasyon olarak en çok üretrovajinal erozyonlar görülmektedir. Bu çalışmada, 78 yaşındaki kadın hastada, stres inkontinans tanısı ile vajinal ön duvar bant ameliyatı sonrası oluşan ve çok nadir görülen enterokutanöz fistül olgusu sunulmaktadır. Fistül düşük debili olduğundan, oral gıda alımı kesilip total parenteral beslenme ile tedavi edilmiştir. Bu olguyu sunma amacımız, çok ender görülen bir komplikasyon olması yanında, bu ameliyatların sanıldığı kadar basit ve güvenilir olmadığını, kullanılan mesh ve sütür materyallerinin çeşitli komplikasyonlara neden olabileceğini, bundan dolayı ameliyattan sonra bu hastaların uzun süre takip edilmeleri gerektiğini vurgulamaktır.
ABSTRACT
The use of synthetic materials in patients with stress type urinary incontinance has greatly increased in the last few years. Similarly, complications related with the use of these materials have also increased. The most common complication seen after bant surgery is occurrence of urethrovaginal erosion. We present a case of an enterocutaneous fistula in a 78 years old lady that developed after anterior vaginal wall bant procedure which was performed due to stress urinary incontinence. We successfully managed this low volume fistula conservatively by cessation of oral intake and administering total parenteral nutrition. Our aim in presenting this case is to emphasize that anterior vaginal wall bant operations by using synthetic materials are not very simple and safe procedures that might lead to various complications such as formation of enterocutaneous fistula. Therefore, we recommended long term postoperative follow-up in these patients.
