Evaluation of Endocrine and Metabolic Parameters in Mothers and Sisters of Patients with Policystic Ovary Syndrome

ÖZET
Amaç: Metabolik bir sendrom olarak kabul edilen ve üreme çağındaki kadınlarda en sık rastlanan endokrin patoloji olan polikistik over sendromu (PKOS); Diabetes mellitus (DM) ve koroner arter hastalığı (KAH) gibi uzun dönem sağlık problemlerinin de eşlik ettiği ve patogenezde genetik faktörlerin önemli rol üstlendikleri bir sendromdur. Bu çalışmada, PKOS'lu hasta ve yakınlarında ileride gelişebilecek kardiyovasküler hastalık (KVH) ve tip 2 DM için var olan risk faktörlerini saptanması ve prediktif yöntemlerin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntemler: Çalışma; Haziran 2008 ile Kasım 2008 tarihleri arasında SB Göztepe Eğitim ve Araştırma Hastanesi Kadın Hastalıkları ve Doğum Polikliniğine başvuran PKOS'lu hastaların 14'ü kız kardeş, 15'i anne olmak üzere toplam 29 hastalık çalışma grubu ve rastgele seçilen 30 hastalık kontrol grubu olmak üzere 59 olgu üzerinde yapılmıştır. Hastaların antropometrik ölçümleri, açlık glukoz, açlık insülin, OGTT, total kolesterol, trigliserid, HDL, LDL, LH, FSH, östradiol, total testosteron, DHEASO4 ölçümleri alınarak her iki grup arasında ve çalışma grubunu oluşturan anne ve kız kardeşlerde KVH ve Tip 2 DM artmış risk faktörleri değerlendirildi. Bulgular: Polikistik over sendromlu hastaların annelerinin %46'sında, kız kardeşlerinin %14'ünde metabolik parametrelerinin bozulduğu izlenmiştir. Kız kardeşlerde açlık glikoz ve total kolesterol düzeyleri, annelerde ise bel çevresi, kilo, serum trigliserid düzeyleri anlamlı düzeyde yüksek bulunmuştur. Sonuç: Çalışmamızda; PKOS'lu hasta yakınlarında obezite, glikoz intoleransı ve lipid metabolizması bozulduğu gösterilmiştir. Annelerde vücut kitle indeksinde bozulma ve dislipidemi ön planda iken kız kardeşlerde glikoz intoleransı dikkat çekicidir.
ABSTRACT
Objective: Polycystic ovary syndrome (PCOS), accepted as a metabolic syndrome is the most common endocrine disorder among women of reproductive age and long term health problems such as diabetes mellitus (DM) and coronary heart disease can accompany to it. There is no doubt about the important role of genetic factors in the pathogenesis of PCOS, nevertheless it seems to be a disorder resulting from interaction of multiple gene and environmental factors. In our study we aimed to achieve; the necessity of investigating metabolic parameters as a routine screening test in the first degree relatives of PCOS patients and early diagnosis and awareness of insulin resistance, hyperandrogenism and hyperlipidemia that can develop in the future. Material and Methods: Our study included 59 patients attending to SB Göztepe Training and Research Hospital Obstetrics and Gynecology outpatient clinics between June 2008 and November 2008. 29 of them were mothers and sisters of PCOS patients and 30 of them were in the control group. Anthropometric measurements, fasting blood glucose levels, fasting blood insulin levels, OGTT, total cholesterol, HDL, LDL, triglyceride, LH, FSH, estradiol, total testosteron, DHEASO4 levels of patients were taken. Results: Metabolic parameters were shown to be abnormal in 46% of mothers of polycystic ovary patients and in 14% of their sisters. Conclusion: Our study shows the genetic contribution of disease, however this relation increases within advancing reproductive ages. In our study body mass index and waist circumference from antropometric measurements was found to be statistically significant in the study group compared to control group. This shows the increasing incidence of obesity in the relatives of PCOS patients.
