ÖZET
Amaç: Polikistik over sendromu (PKOS) halen etyoloji, tanı ve tedavi açısından tam netlik kazanmamış bir hastalıktır. Bu çalışmada iki farklı tedavi ile PKOSda ovaryan morfolojide oluşabilecek değişikliklerin manyetik rezonans görüntüleme ile değerlendirilmesi amaçlandı. Çalışmanın Yapıldığı Yer: İnönü Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum AD. Materyal ve Metod: Jinekoloji polikliniğine başvuran klinik, biyokimyasal ve USG özellikleri PKOSa uyan 34 hasta çalışmaya alındı. Dört hasta çalışma dışı kaldı, değerlendirme 30 hasta üzerinden yapıldı. Bulgular: Üç aylık tedaviyi takiben kontrol MRI uygulandı. İlk grupta korteks kalınlığında ikinci grupta ise over volümünde anlamlı azalma kaydedildi. Hastalar iki eşit gruba ayrıldı. Hepsinden ovaryan MRI istendi. Ardından ilk gruba etinil estradiol-dezogestrel (Desolett), ikinci gruba etinil estradiol-siproteron asetat (Diane-35) kombinasyonu başlandı. Sonuç: Bu çalışma ile PKOS tanısında ve tedavinin takibinde MRIın çok objektif bir inceleme yöntemi olduğu sonucuna varıldı. Ayrıca tedavi için kullanılan kombinasyonların over morfolojisinde değişikliğe neden olarak altın standart olmaya devam ettikleri sonucu çıkarıldı.
ABSTRACT
Objective: Polycystic ovarian syndrome (PCOS) is a much-debated topic, for which treatment and diagnostic criteria are not fully established nor pathophysiology clearly understood. The pur-pose of this study is to evaluate the changes of ovarian mor-phology in PCOS with two different medications and to assess the response with magnetic resonance imaging. Institution: University of Inonu School of Medicine Department of Obstetrics and Gynecology, Malatya. Materials and Methods: Thirty-four patients with diagnosis of PCOS by clinical, biological and ultrasonographic examina-tions were included in the study. Four of them were ex-cluded and 30 patients were evaluated. Results: After 3-months of therapy, control MRI was performed. There was significant decrease in ovarian cortex thickness in the first group and ovarian volume in the second group. Pa-tients were divided in to two groups. MRI was performed to all of them. And than the combination of etinil estradiol-desogestrel (Desolett) was given to the first group and etinil estradiol-cyproterone acetate (Diane-35) was given to the second group. Conclusion: As a result, we concluded that MRI maping is very objective procedure to diagnose and follow up the treatment of PCOS. Moreover the combinations that we use for the treatment restores the ovarian morphology and they con-tinue to be the gold standart for treatment of PCOS.
