Fetal Intracranial Cystic Lesions Detected in Two Cases: Diagnosis, Follow-Up and Prognosis

ÖZET
Prenatal dönemde saptanan fetal intrakraniyal hipoekoik lezyonların nedenleri arasında, araknoid kistler ve vasküler lezyonlar bulunmaktadır. Ayırıcı tanının yapılmasında, Doppler ultrasonografi önemlidir. İlk olguda, 28. gebelik haftasında, rutin fetal ultrasonografide, 15 x 20 mm boyutlarında araknoid kist saptandı. Takibinde kist boyutlarında artış ve ventrikülomegali izlendi. Termde doğurtulan hastaya postnatal 41. günde, ventriküloperitoneal şant uygulandı. Yedinci ayda, hastanın semptomsuz ve nörolojik gelişiminin normal olduğu izlendi. İkinci olgu, 34. gebelik haftasında, rutin fetal ultrasonografide, intrakraniyal hipoekoik lezyon izlenmesi nedeni ile kliniğimize refere edildi. Renkli Doppler ultrasonografide, fetal kraniyumda galen ven dilatasyonu saptandı. Termde doğurtulan hastanın izleminde medikal tedaviye dirençli kardiyak yetmezlik gelişti ve infant 4. günde kaybedildi.
ABSTRACT
Fetal intracranial hypoechoic lesions are caused by arachnoid cysts or vascular malformations. Doppler ultrasound is important for differential diagnosis. In first case, a routine obstetric ultrasound examination in 28 weeks gestation revealed a 15 x 20 mm arachnoid cyst. There was an increase in the size of the cyst and ventriculomegaly was developed during the course of the pregnancy. The baby was delivered at term and a ventriculoperitoneal shunt inserted on day 41 of life. At the age of 7 months, the patient had no symptoms and neurological development was normal. The other case was referred to our hospital because of the hypoechoic intracranial lesion on routine obstetric ultrasound examination at 34 weeks gestation. Color Doppler investigation demonstrated dilatation of the vein of Galen. The baby was delivered at term. Congestive heart failure resistant to medical treatment developed during the clinical course and the infant died 4 days after delivery.
