ÖZET
Amaç: Fetal akciğer matürasyonunun değerlendirilmesinde kullanılan Tap testinin tanısal değerini saptamak ve Shake testi ile karşılaştırmak. Çalışmanın Yapıldığı Yer: Osman Gazi Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum AD. Materyal ve Metod: Prematür eylem veya 12 saati geçmeyen prematür membran rüptürü nedeniyle kliniğimize başvuran ve 28-40 gebelik haftasında olan ve 72 saat içinde doğum yapan 74 gebede fetal akciğer matüritesi tayin edildi. Amniotik sıvı transvaginal, transabdominal amniosentez ile veya sezaryen esnasında alındı. Bu amniotik mayiden shake ve tap testi yapıldı. Bebekler respiratuar distress sendromu yönünden neonatal dönemde klinik ve radyolojik olarak değerlendirildi. Bulgular: Otuzyedi haftanın üzerinde doğan bebeklerin hiçbirinde respiratuar distress sendromu görülmedi. Olguların %37.8'i termden önce doğum yaptı. Tap testinin fetal akciğer matürasyonunu tesbit etmede, 2.5 ve 10. dakika pozitif prediktif değerleri sırasıyla %98.1, %96.3, %96.6 negatif prediktif değerleri %52.3, %52.6 ve %66.6 olarak bulundu. Shake testinin pozitif prediktif değeri %97.2, negatif değeri ise %52.9 idi. Shake testinin 2. dakika tap testi ile uyumluluğu %83.3, 5. dk. tap testi ile uyumluluğu %87.0, 10. dk. tap testi ile uyumluluğu %85.1 olarak bulundu.
ABSTRACT
Objective: To evaluate the diagnostic value of Tap test in relation to Shake test in the assessment of the fetal lung maturity. Institution: Osman Gazi University Faculty of Medicine, Department of Obstetrics and Gynecology. Material and Methods: Presence of fetal pulmonary maturity was investigated in 74 pregnant patients who were admitted to clinic because of either preterm labor or premature rupture of membranes (less than 12 hours) and delivered in 72 hours during 28 to 40 gestational weeks. Amniotic fluid was aspirated transvaginally, via transabdominal amniocentesis of during cesarean section. Shake and tap test were performed in each case. Clinical and radiological signs of respiratory distress syndrome were noted in neonatal period. Results: Respiratory distress syndrome was not detected in the babies who delivered after 37. weeks of gestation. Preterm delivery was encountered in 37.8% of the cases. Positive predictive values of the tap test in second, fifth and tenth minutes were 98.1%, 96.3% and 96.6% respectively. Positive predictive value of the shake test was 97.2% and negative predictive value was 52.9%. Shake test was correlated with second, fifth and tenth minute tap test in 83.3%, 87.0% and 85.1% of the cases, respectively. Conclusion: Diagnostic value of the tap test is at least as effective as the shake test in detection of fetal lung maturity.
