ÖZET
Amaç: Prognozu bilinen normal gebeliklerde 16-24. gebelik haftalarında fetusun serebral lateral ventriküler atriyum genişliği (LVG)'nin ve serebral LVG'nin biparyetal çap (BPD)'a oranının dağılımının değerlendirilmesi ve nomogramının elde edilmesi. Gereç ve Yöntemler: Bu prospektif çalışmada, 16-24. gebelik haftalarında yapısal olarak normal 734 fetusa ait fetal biyometrik ölçümler transabdominal ultrason ile yapıldı. Bu fetusların LVG'nin gebelik haftalarına göre dağılımı çıkarıldı ve 16-24. gebelik haftaları arasında yüzdelik değerleri hesaplandı. LVD'nin, gebelik haftası (GH) ve BPD ile regresyon analizleri yapıldı. LVG/BPD'nin gebelik haftalarına göre dağılımı çıkarıldı ve gebelik haftası ile regresyon analizi yapıldı. Bulgular: 16-24 gebelik haftaları arasında LVG'nin medyan değerleri 16. haftadan 24. haftaya sırasıyla 7.0-7.3-7.2-7.7-7.0-6.7-6.3-6.2 ve 6.2 mm olarak saptandı. LVG'nin gebelik haftası ile progresif olarak artmadığı saptandı. Ortalama LVG 6.73 mm ± 1.08 (ortalama ± standart sapma) olarak bulundu. Medyan, LVD/BPD'de 16. haftadan 24. haftaya sırasıyla 0.21-0.20-0.18-0.17-0.15-0.13-0.12-0.11 ve 0.11 olarak saptandı. Bu oranın gebelik haftası (GH) ile ilişkisi incelendiğinde, 16-24 gebelik haftaları arasında lineer olarak azaldığı ve LVD/BPD ile GH arasındaki lineer regresyon analizi ile regresyon denklemi: LVD/BPD= 0.425-GH x 0.013 saptandı. (r2= 0.68; p< 0.001). Sonuç: LVG'nin gebelik haftası ile progresif olarak artmadığı saptandı.
ABSTRACT
Objective: To obtain nomogram and normal range of fetal cerebral lateral ventricular atrial diameter (LVD) and the ratio of fetal cerebral LVD to biparietal diameter (BPD) at 16-24 weeks of gestation in known prognosis of normal pregnancies. Material and Methods: Fetal LVD and other routine biometric measurements of 734 structurally normal fetuses at 16-24 weeks of gestation were measured prospectively by transabdominal ultrasound. The distribution of LVD is established according to gestational weeks (GW) and percentiles between 16-24 weeks are calculated. Relationship between fetal LVD; gestational weeks and biparietal diameter were assessed by regression analysis. The distribution of LVD/BPD ratio is established according to gestational weeks and assessed by regression analysis. Results: Median values of LVD between 16-24 weeks were 7.0-7.3-7.2-7.7-7.0-6.7-6.3-6.2 and 6.2 mm, respectively. The LVD did not progressively increase with advancing gestational age. The mean LVD for the combined group of all 734 cases was 6.73 mm ± 1.08 (mean ± standard deviations). Median values of LVD/BPD between 16-24 weeks were 0.21-0.20-0.18-0.17-0.15-0.13-0.12-0.11 and 0.11, respectively. The ratio of LVD/BPD is linearly decreased between 16-24 weeks. Significant correlation was also found between the ratio of LVD/BPD and GW (LVD/BPD= 0.425-GW x 0.013 (r2= 0.68; p< 0.001)). Conclusion: The LVD did not progressively increase with advancing of gestational age.
