ÖZET
Amaç: Bu vaka sunumunun amacı, ailesel hipertrigliseridemisi ve akut pankreatiti olan bir gebenin sunulmasıdır.Olgu Sunumu: 23 yaşında 22. gebelik haftasında olan hasta bulantı, kusma ve epigastrium ağrısı ile Selçuk Üniversitesi Meram Tıp Fakültesi Kadın Hastalıkları ve Doğum Kliniğine başvurdu. Hastanın ailesinde hipertrigliseridemiye bağlı geçirilmiş akut pankreatit öyküsü mevcuttu. Labaratuvar bulguları, beyaz küre 18.000 K/uL, hemoglobin 14.1g/dL, sedimantasyon hızı 88 mm/saat, glukoz 135 mg/dL, total kolestrol 409 mg/dL, trigliserit 1583 mg/dL, amilaz 134 u/L, lipaz 190 u/L, SGOT 50 u/L, SGPT 48 u/L, LDH 484 u/L, kalsiyum 11.6 mg/dL, serum amilaz/kreatinin klirensi oranı %5 idi. Lipid profilinde sadece VLDL de artış görüldü. Hastaya medikal tedavi uygulandı ve fetal-maternal mortalite ve morbidite gözlenmedi. Sonuç: Ailesel hipertrigliseridemili olgularda gebelikte akut pankreatit görülebilir ve bu olgular medikal tedavi ile başarılı bir şekilde tedavi edilirler.
ABSTRACT
Aim: The aimed of this case report is to present a pregnant woman who had familial hypertrigliseridemia and acute pancreatitis.Case Report: A 23 years old patient at twenty-second gestational weeks was admited to Selçuk University, Faculty of Meram Medicine, Departments of Obstetrics and Gynecology with nausea, vomiting, and epigastric pain. Patients family has an acute pancreatitis history due to hypertriglyceridemia. Laboratory findings were leukocyte 18.000 K/uL, hemoglobin 14.1g/dL, sedimentation rate 88 mm/ hour, glucose 135 mg/dL, total cholesterol 409 mg/dL, triglycerid 1583 mg/dL, amylase 134 u/L, lipase 190 u/L, SGOT 50 u/L, SGPT 48 u/L, LDH 484 u/L, calcium 11.6 mg/dL, serum amylase/ creatine clearance ratio: 5%. Increasing VLDL only was seen at the lipid profile. Medical therapy was used without fetal and maternal morbidity and mortality.Result: Acute pancreatitis might have seen in cases with familial hypertrigliceridemia at pregnancy and these cases can be treated with medical therapy successfully.
