ÖZET
Bu olgu raporunda nadir görülen 18 hafta gebe herpes gestasyones olgusu sunulmuş olup ilgili literatür kısaca gözden geçirilmiştir. Karın, kol ve bacaklarda kaşıntılı deri lezyonları nedeniyle dermatoloji polikliniğine başvuran 24 yaşındaki, 18 haftalık gebeliği olan hastanın yapılan dermatolojik muayenesinde karın, kol ve bacaklarda ürtikaryal papül ve plaklar, grup yapmış papüloveziküler lezyonlar ve erozyonlar görüldü. Histopatolojik değerlendirme için sağ kolda lezyonlu alandan insizyonel biyopsi yapıldı. Makroskobik olarak 0.6x0.2x0.2 cm boyutlarında grimtrak kahve renkte, bir yüzünde cilt bulunduran doku parçasının mikroskobik incelenmesinde; epidermiste spongiozis, bazı alanlarda intrasellüler ve subepidermal bül oluşumu, bazı papillalarda ödem ve üst dermiste damarlar çevresinde daha yoğun olmak üzere monosellüler ve eozinofillerden oluşmuş inflamatuar infiltrat görüldü. Bu klinik ve histopatolojik bulgularla olguya herpes gestasyones tanısı kondu. Herpes gestasyonis gebeliğin ikinci veya üçüncü trimesterinde başlayan, nadir, otoimmün büllöz bir dermatozdur. Doğumdan haftalar sonra iyileşme olur. Sonraki gebeliklerde daha şiddetli şekilde ortaya çıkar. Temel tedavisi sistemik steroid ve antihistaminik uygulanmasıdır.
ABSTRACT
The aim of this study was to present the rare case of herpes gestationis at 18 weeks' of gestation and to review related literature. A 24-year-old at 18 weeks' of gestation admitted to department of dermatology with the complain of itchy skin lesions on the bell, arms and legs. Local examination revealed urticarial papula and plague, papulovesicular lesions which were grouped and erosions on the area of the bell, arms and legs. An incision biopsy was performed from the lesions located on right arm. Macroscopic examination revealed a gray-brown colored tissue with 0.6x0.2x0.2 cm dimensions while microscopic examination confirmed the findings of spongiosis on epidermis, intracellular and subepidermal bullae formation, edema on papillary architecture and inflammatory infiltrate composed of mononuclear cells and eosinophils around the perivascular ares of the upper dermis. The preliminary diagnosis of herpes gestationis was confirmed by histopathologic findings. Herpes gestationis is a rare autoimmune bullous dermatosis which in mostly seen in the second or the third trimester of pregnancy. Recovery usually occurs in several weeks of giving birth. Recurrence of this rare condition is more severe in later pregnancies. Systemic steroid and antihistaminics are the choice of treatment.
