Pregnancy In Menopause During Hormone Therapy: A Case Report

ÖZET
Menopoz, ovarian aktivite kaybını takiben mensturasyonun kalıcı duraklaması olarak tanımlanabilir ve yaştan bağımsız olarak over foliküllerinin yaklaşık 1000'in altına düşmesi ile gerçekleşir. Literatürde 'prematür over yetmezliği ve gebelik' görülmekle beraber, 40 yaşından sonra menopoz nedeni ile Hormon Tedavisi (HT) alan hastalarda gebelik olgusu oldukça nadirdir. 42 yaşında 8 aydır adet görmeyen ve hormon profili menopoz ile uyumlu olan hastaya 0,625 mg konjuge östrojen ve 5 mg medroksiprogesteron asetat (premelle cycle) HT amacıyla verilmiştir. 10 ay tedaviye devam ettikten sonra bir ay ara veren hastada 10 haftalık canlı gebelik saptanmıştır. Kasım 2003 tarihinde sezaryen ile 4050 gr ağırlığında sağlıklı kız bebek doğuran ve tüp ligasyonu yapılan hastanın Nisan 2004 tarihinde, laktasyonun olmadığı dönemde tekrarlanan hormon profili menopoz ile uyumlu (FSH: 78 IU/L, LH:51 IU/L, E2:<20 pg/ml) bulunmuştur. Yüksek gonadotropin seviyelerinin HT alan hastalarda normale dönebileceği ve menopozun ilk yıllarında gebelik oluşabileceği ihtimalini göz önünde bulundurmak gerekir.
ABSTRACT
The menopause is that point in time when permanent cessation of menstruation occurs following the loss of ovarian activity and occurs when the number of remaining follicles falls below a critical threshold, about 1000, regardless of age. Although premature ovarian failure (POF) and pregnancy could be mentioned together in the literature, the pregnancy of a woman at 40's age having hormone therapy (HT) because of menopause is very seldom. A forty-two year old woman with a complaint of eight month amenorrhea and hormone profile representative menopause, had been given 0.625mg conjugated estrogen and 5mg medroxyprogesteron acetate (premelle cycle) as HT. After having this medication for10 months, she discontinued it for a month and a live 10 weeks pregnancy was occured. In November 2003 she had a 4050gr, healthy, female baby by cesarean section and tubal ligation at the same operation.The repeated hormone profile in April 2004 was also interpreted as she was in menopause (FSH: 78 IU/L, LH:51 IU/L, E2:<20 pg/ml). It should be considered that elevated gonadotropin levels could return to normal levels during HT and that pregnancies could be seen in the first years of menopause .
