A Single Umbilical Artery In A Pregnant Patient After An Icsi Cycle With Cryopreservation And Embrio Transfer: A Case Report

ÖZET
Giriş: Umbilikal kordonun en sık gözlenen anomalisi tek umbilikal arter varlığıdır. Doğumların %0.2-2.1 ile otopsilerin %2.7-12sinde bildirilmiştir. Olgu: 37 yaşında ve 16 yıllık primer infertil hastanın superovulasyon sonrası elde edilen metafaz-2 oositlerine ICSI uygulandı. Embriyolar, ovarian hiperstimülasyon sendromu gelişmesi üzerine donduruldu, yapay siklustan sonra transfer edildi ve tekil gebelik oluştu. Perinatal takiplerinde, 18inci haftada Doppler ultrasonografide tek umbilikal arter saptandı. Fakat kapsamlı ultrasonografik incelemelerde ek anomali saptanmadı. 39uncu haftada sezaryen operasyonu ile 3400 gr, 52 cm canlı erkek doğurtuldu. Bebekte patoloji saptanmadı. Sonuç: Sonografide tek umbilikal arter saptanması, fetüsün diğer anomaliler yönünden tam olarak incelenmesini gerektirir. Birlikte görülen anomalilerin sıklığı bir çok çalışmada %25-50 arasındadır.
ABSTRACT
Introduction: A single umbilical artery is the most common abnormality of the umbilical cord. It is detected in about 0.2-2.1% deliveries and 2.7-12% of autopsies. Case: A 37 year old patient who had infertility problems for 16 years, underwent superovulation and ICSI was performed to the metaphase-II oocytes. The embryos were cryopreserved due to the risk of ovarian hyperstimulation. In a natural cycle embrios were transfered and the patient concieved. Doppler duplex sonography revealed a single umbilical artery at the 18th week of gestation, however, there was no additional sonographic abnormality. Ultimately, the patient delivered a healthy baby with 3400gr weight and 52cm length at the 39th week of gestation by sectio. Result: It is necessary to screen foetus for additional abnormalities who have single umbilical artery. Many studies showed that almost 25-50% of the patients also have other abnormalities.
