ÖZET
Amaç: Vajinal kanaması olan ilk trimester gebeliklerinde prognozun belirlenmesinde CA-125 ve b-HCG ölçümlerinden faydalanılıp faydalanılamayacağını değerlendirmek. Çalışmanın Yapıldığı Yer: Zekai Tahir Burak Kadın Sağlığı Eğitim ve Araştırma Hastanesi antenatal takip kliniği ve acil servisi. Materyel ve Metod: Bu çalışma Nisan 1997- Temmuz 1997 döneminde yapıldı. Çalışma grubu olarak acil polikliniğe başvurmuş, gebelik haftaları 5 ile 12 arasında değişen 35 hasta, kontrol grubu olarak acil servise başvurmuş olan ve gebelik haftaları yine 5 ve 12 arasında değişen 15 hasta araştırma kapsamına alındı. İstatistiksel analiz için Kruskal Wallis tek yönlü varyans analizi ve Mann-Whitney U testleri kullanıldı. Bulgular: Kontrol grubu ve kanaması olup abortus yapmayan gebe grubu ile kanama sonrası abortus yapan hasta grubunun CA-125 ve b-HCG değerleri ortalamaları arasında istatistik olarak anlamlı farklılık bulundu (CA-125 için 18.60 ve 26.75e 51.50, b-HCG için 95.239 ve 86.499a 18.534 ). Sonuç: Bu çalışmanın sonuçları, abortus imminens vakalarında prognostik belirlemenin önemli olduğu durumlarda biyokimyasal markerlarla klinik değerlendirmeyi korele etmeyi amaçlayan çalışmaların geçerliliğini desteklemektedir.
ABSTRACT
Objective: Determining the usefulnes of CA-125 and b-HCG blood levels to predict the prognosis of the first trimester pregnancies complicated by vaginal bleeding. Institution: Antenatal clinic and emergency care unit of Zekai Tahir Buraks Hospital of research and education on womens health. Materials and Method: This study is conducted between april and july the 1997 . the study group was comprised of 35 pregnant patients who is applied to emergency deparment by the symptom of vaginal bleeding. Their pregnancies were all in 5 to 12 weeks range. Fifteen patients whose pregnancies were in the same range and who applied to the antenatal clinic for routine examination were chosen as the control group. For statistical analysis Kruskal-Wallis one way anova and Mann-Whitney U tests were used. Results: The differences between mean CA-125 and b-HCG values between the patient group who aborted after the bleeding and both non-aborted and control groups were found statistically important (for CA-125; 51.50, 18.60 and 26.75, for b-HCG; 18.534, 86.499 and 95.239 i.u. respectively). Conclusion: The results of this study correlates with the results of the studies which aim to use both clinic judgement and biochemical markers to predict the prognosis in threatening abortion cases.
