ÖZET
Amaç: Doğum eylemi sırasında tespit edilen anormal fetal kalp hızı paternlerinin fetal asidozu belirlemedeki değerini araştırmak.Çalışmanın yapıldığı yer: Kocaeli Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim DalıMateryal metod: Çalışma 1 Ocak 2001 ile 30 Mayıs 2001 arasında Kocaeli Üniversitesi Araştırma ve Uygulama Hastanesine doğum yapmak üzere başvuran 38-42 haftalar arasındaki 124 gebede gerçekleştirildi. Gebeler doğum esnasında fetal kardiotokogram ile sürekli moniterize edildi. Variabilite kaybı, geç deselerasyon, variabl deselerasyon anormal fetal kalp hızı paternleri olarak kabul edildi. Doğum sonrası 1 ve 5. dakika Apgar skor ile ve umblikal arter pH ve baz açığı tayini ile fetal asidoz değerlendirildi. Birinci ve beşinci dakika Apgar skor <7, umblikal arter pH<7.1, umblikal arter baz açığı >-14 fetal asidoz bulgusu olarak kabul edildi. Doğum esnasında elde edilen anormal fetal kalp hızı paternlerin fetal asidozu belirlemedeki değeri araştırıldı.Bulgular: Fetal kalp hızı paternlerinin Apgar skor düşüklüğünü belirlemedeki sensitivite, spesifite, pozitif prediktif değer, negatif prediktif değeri birinci dakika için sırasıyla %100, %89.2, %46.4 ve %100, beşinci dakika için sırasıyla %100, %82.6, %7.1 ve %100 bulundu. Anormal fetal kalp hızı paterninin umblikal arter pHsının <7.1 olmasını belirlemedeki sensitivitesi %60, spesifitesi %82.9, negatif prediktif değeri %98.3, pozitif prediktif değeri %10.7 olarak tespit edildi. Anormal kalp hızı paternlerinin fetal asidozun önemli bir belirteci olan umblikal arter baz açığını belirlemedeki sensitivitesi %80, spesifitesi %83.6, pozitif prediktif değeri %14, negatif prediktif değeri %99.1 olarak tespit edildi.Sonuç: Travay esnasında normal kalp atım hızı paternlerinin tespiti, yalancı negatifliğinin düşük olması nedeniyle sağlıklı bir bebeğin doğumuyla birliktelik gösterir. Ancak anormal kalp hızı paternlerinin tespit edilmesinin yalancı pozitifliğinin yüksek olduğu göz önüne alınmalı ve anormal fetal kalp atım hızı paternlerinin fetal asidoz olarak yorumlanmasının sezaryen oranlarında artışa neden olabileceği unutulmamalıdır.
ABSTRACT
Objective: The purpose of this study was to investigate the value of abnormal fetal heart rate patterns during labor in predicting fetal acidosis.Institution: University of Kocaeli, School of Medicine, Department of Obstetrics and GynecologyMaterials and Methods: A prospective study consisted of 124 patients at 38-42 weeks of pregnancy who admitted to the Obstetric Unit of Kocaeli University Hospital between January 1 2001 and May 30 2001. Continuous fetal heart rate monitoring during labor was recorded and those having decreased variability, variable and late decelerations were considered as abnormal heart patterns. After delivery, Apgar scores at 1 and 5 minutes, umbilical artery pH and base deficit values were recorded to predict fetal acidosis. Apgar scores less than 7, umbilical artery pH less than 7.1 and base deficit > -14 were accepted as the result of fetal acidosis. We investigated the value of abnormal fetal heart rate patterns during labor in predicting fetal acidosis.Results: The sensitivity, specifity, positive predictive value, negative predictive value of abnormal fetal heart rate patterns for predicting low 1 minute Apgar score were 100%, 89.2%, 46.4% and 100%, respectively. The sensitivity, specifity, positive predictive value, negative predictive value of abnormal fetal heart rate patterns for predicting low 5 minute Apgar score were 100%, 82.6%, 7.1%, 100%, respectively. The sensitivity, specifity, positive predictive value, negative predictive value of abnormal fetal heart rate patterns for predicting umbilical artery pH< 7.1 were 60%, 82.9%, 10.7%, and 98.3%, respectively. The sensitivity, specifity, positive predictive value, negative predictive value of abnormal fetal heart rate patterns for predicting umbilical artery base deficit were 80%, 83.6%,14%. and 99.1%, respectively.Conclusion: Since the normal fetal heart rate tracings during labor has a low false negativity rate, it is almost always accompanies with the birth of a healthy infant. On the other hand, the obstetrician must be aware of the high false positivity rate of abnormal fetal heart rate patterns which might unnecessarily increase cesarean rates.
