Ischemia-reperfusion Damage In Ovary And Effects Of Melatonin(Experimental Study In Rats)

ÖZET
Amaç: Adneks detorsiyonu ile overlerde oluşturulan iskemi reperfüzyon hasarı ve melatoninin etkisini araştırmak. Gereç ve Yöntem: Çalışmada dört gruba ayrılan toplam 28 wistar tipi dişi rat kullanıldı. Tüm ratlarda adneks torsiyonu yapıldı. Grup I (n:6)de 24 saatlik iskemiden sonra detorsiyon (reperfüzyon) yapılmadan salpingooferektomi yapıldı. Grup II (n:6)de adneks detorsiyone edildikten sonra herhangi bir uygulama yapılmadı. Grup III (n:8)de reperfüzyon yapıldıktan sonra 2 cc hacimde 100mg/kg Melatonin (MLT) (Sigma Chemical Co), GrupIV (n:8)de ise aynı hacimde serum fizyolojik (SF) dört eşit dozda ve 6 saat arayla (0., 6., 12., 18. saatlerde) intraperitoneal (İP) uygulandı. Detorsiyon işleminden 24 saat sonra tüm ratlar dekapite edilerek serum malondialdehit (MDA) düzeyi ve over histopatolojisi yönünden karşılaştırıldı. İstatistiksel yöntem olarak Mann Whitney U testi kullanıldı ve p<0.01 anlamlı kabul edildi. Histopatolojik bulgular ve MDA arasındaki bağıntı incelemesinde spearman bağıntı analizi (r,p,n) kullanıldı.Bulgular: Grup I histopatolojik skorları (HPS) 5.3 ± 0.5, MDA düzeyleri 0.38±0.10, Grup IIde HPS 7.5 ± 1, MDA düzeyleri 0.58 ± 0.1 olarak bulundu. HPS ve MDA düzeyleri yönünden aralarında anlamlı fark bulundu (sırasıyla p=0.005 ve p=0.004). Grup IIIde HPS 5.8 ± 0.8, MDA düzeyi 0.48 ± 0.08 olarak bulunurken GrupIVde HPS düzeyi 7.3 ± 1, MDA düzeyi 0.60 ± 0.05 olarak tespit edildi. HPS ve MDA yönünden aralarında anlamlı fark bulundu (sırasıyla p=0.001, p=0.002). MDA düzeyleri ile HPS arasında anlamlı bağıntı bulundu (r:0.71, p:0.00,n:28).Sonuç: Adneksiyal torsiyon nedeniyle detorsiyon yapılan durumlarda overlerde oluşabilecek iskemi - reperfüzyon hasarında tedaviye melatonin eklenmesi faydalı olabilir.
ABSTRACT
Objective: It was purposed to examine the effects of melatonin supplementation on ischemia-reperfusion damage in ovary induced by adnexal detorsion. Material and Methods: In the present study 28 female wistar rats were used in which divided 4 groups. Adnexal torsion was created by twisting the adnexa to all of the rats. In Group I (n=6): adnexes were extirpated without detorsion (reperfusion), following 24 hours ischemia period. In Group II (n=6): only adnexal detorsion was created without saline and melatonin application. In Group III (n=8): After reperfusion, 2 cc volume containing 100mg/kg melatonin (MLT) (Sigma Chemical Co) was injected intraperitonealy, in Group IV (n=8): 2 cc volume of saline was divided in four equal doses and injected intraperinoneally every 6 hours (0., 6., 12., 18. hours). Following 24 hours reperfusion period, all the rats were decapitated and their serum malonyldialdehide (MDA) levels and ovary histopathological examination were compared. Statistical significance between the groups were examined by Mann Whitney U test (p<0.01). The correlation between MDA levels and the score of histopathologic examination were made Spearman Correlation Analysis (r,p,n) method.Results: The histopathologic score (HPS) and MDA levels in Group I were 5.3 ± 0.5, 0.38±0.10 nmol/ml, respectively. In Group II, HPS and MDA levels were found 7.5 ± 1 and 0.58 ± 0.1 nmol/ml, respectively. There was statistically significant differences between HPS and MDA levels (p=0.005 and p=0.004, respectively). In Group III, HPS was 5.8 ± 0.8 and MDA level was 0.48 ± 0.08 nmol/ml. In Group IV, HPS and MDA levels were found to be 7.3 ± 1 and 0.60 ± 0.05 nmol/ml, which were statistically different from the group III (p=0.001, p=0.002, respectively). In addition, the correlation between MDA and HPS was statistically significant (r=0.71, p=0.00, n:28).Conclusion: The results of present study show that the melatonin supplementation has an important effect on prevention of ischemia-reperfusion damage in ovary induced by adnexal detorsion.
