Perinatal Outcomes In Pregnant Women With Premature Rupture Of Membranes In Our Clinic

ÖZET
Amaç: Erken membran rüptürü (EMR) yenidoğanın morbidite ve mortalitesini önemli derecede etkilemektedir. Bu çalışmada EMR nedeniyle takip edilen gebelerde perinatal sonuçlar incelendi. Gereç ve Yöntemler: Çalışmaya 110 hasta dahil edildi. Hastaların vajinal kültür, ultrason, lökosit, sedimentasyon, ateş, CRP tetkikleri yapıldı. Hastalar 26 hafta altı (1. grup), 27-36 hafta arası (2. grup), 37 hafta üzeri (3. grup) olmak üzere 3 grupta incelendi. 34 haftanın altındaki gebelere betametazon ve IV ritodrin tedavisi uygulandı. Tedaviye cevap alınamayan vakalar sezaryen ya da normal doğumla doğurtuldu. Bulgular: EMR insidansı %4.8 olarak tesbit edildi. Hastaların 12 (%10.9)'si 1. grupta, 67 (%60.9)'si 2. grupta, 31 (%28.2)'i 3. grupta idi. Hastaların %91.1'i 35 yaş ve altında tesbit edildi. %40.2'sinin ilk gebeliği idi. Vakaların 19 (%17.2)'unda etiyolojide fark edilmemiş doğum başlangıcı, 16 (%14.5)'sında enfeksiyon bulguları, 13 (%11.8)'ünde makat geliş, 12 (%10.9)'sinde ikiz gebelik, 5 (%4.5)'inde transvers duruş tesbit edildi. Latent period 2 saat-25 gün arasında saptandı. CRP %49.1'inde, sedimentasyon %93.7'sinde 20'nin üzerinde idi. Vajinal kültürlerde 4 hastada E. coli, 2 hastada enterekok, 2 hastada stafilokok aureus, 1 hastada neisseria, 1 hastada psödomonas aeroginoza üredi. Hastaların ortalama doğum haftaları 33.3 hafta idi. Hastaların 47 (%42.7)'si ilk 12 saat içinde, 72 (%65.4)'si ilk 24 saatte, 102 (%92.7)'si ilk 1 hafta içinde doğumunu yaptı. 76 (%69) hasta normal doğum ile doğurtuldu. 32 (%29) hastaya oksitosinle indüksiyon, 34 (%30.9) hastaya sezaryen uygulandı. Bebeklerin ortalama doğum kilosu 2150 g idi. 27 bebek eksitus oldu. Bu bebeklerin 19'u 26 haftanın altında idi. Sonuç: EMR perinatal sonuçları etkileyen obstetrik bir patolojidir. Hastaları çok dikkatli bir şekilde değerlendirilerek, gebelik haftası ve enfeksiyon bulguları başta olmak üzere tüm faktörler göz önüne alınarak uygun tedavi yöntemi planlanmalıdır.
ABSTRACT
Objective: Premature rupture of membranes (PROM) effects to neonatal mortality and morbidity. Perinatal outcomes of women with PROM were examined in this study. Material and Methods: Hundred and ten women with PROM were taken into the study and divided into 3 groups as gestational age between 20-26 weeks (1st group); between 27-36 weeks (2nd group) and beyond 37 weeks (3rd group). Patients were followed by vaginal culture, ultrasound, leukocyte, erithrocyte sedimentation rate (ESR), presence of fever and C-Reactive Protein (CRP). Bethametazone and iv ritodrine were given to patients having gestational age less than 34 weeks. Patients that didn't respond to the treatment were delivered vaginally or by C-section. Results: Incidence of PROM was found 4.8%. Twelve patients (%10.9) were in the 1st, 67 (%60.9) were in the 2nd, 31 (%28.2) were in the 3rd group. Age was less than 35 years in 91.1% of patients. 40.2 % of women were primiparous. Unexpected begining of labor was found in 19 (%17.2), infection in 16 (%14.5) patients, breech presentation in 13 (%11.8), multiple pregnancy in 12 (%10.9), transverse presentation in 5 (%4.5) as etiologic cause. Latent period was between 2 hours to 25 days. CRP and ESR were above 20 in 49.1% and 93.7% of patients respectively. E. coli was grown in vaginal culture of 4 patients, enterococci in 2, staphilococcus aureus in 2, neisseria in 1, pseudomonas in 1 patient. Mean delivery week was 33.3 weeks. Delivery was occured in 12 hours in 47 (%42.7) cases, in 24 hours in 72 (%65.4) and in a week in 102 (%92.7) patients. 76 (%69) patients were delivered vaginally. Induction of labour with oxytocin was applied to 32 (%29) cases. 34 (%30.9) patients were delivered by cesarean section. Mean birtweight was 2150 grams. 27 babies were died. 19 of these were smaller than 26 weeks. Conclusion: PROM is an obstetric problem effecting perinatal outcomes. Appropriate treatment must be applied after patients were evaluated carefully, especially about gestational age and presence of infection.
