The Analysis Of Complete Typical HELLP Cases In Our Department

ÖZET
Amaç: Kliniğimize başvuran preeklampsi vakalarındaki komplet tipik HELLP sendromu olgularının klinik, biyokimyasal değişikliklerinin ve maternal, fetal komplikasyonların incelenmesi, literatürle karşılaştırılması.Çalışmanın yapıldığı yer: Fırat Üniversitesi Tıp Fakültesi Kadın Hastalıkları Ve Doğum ABD. Elazığ.Materyal ve Metod: 1 Ekim 1997?1 Ekim 1999 tarihleri arasında kliniğimizde yatırılan 150 preeklamptik hastadan Komplet tipik HELLP sendromu kriterlerini taşıyan gebeler prospektif olarak incelendi. Tüm olgulara daha önce tespit edilen klinik, laboratuar parametreler ve maternal, fetal komplikasyonlar bakıldı. Şiddetli preeklamptik olgularda HELLP sendromu gelişme riski (Relatif Risk) hesaplandı.Bulgular: Yüz elli vakanın 40ı (%26.6) şiddetli, 110u (%73.4) hafif preeklampsi olup, şiddetli preeklamptik vakaların 20sinde Komplet tipik HELLP sendromu (%13.3) teşhis edildi. Şiddetli preeklamptik gebelerde HELLP sendromu gelişme riski, RR=55 bulundu. Olgularımızın önemli biokimyasal parametrelerinin değerleri (mean±SD), SGPT 379±249 IU/L, SGOT 724±689 IU/L, LDH 2109±778 IU/L, hematolojik anormalliklerden ise platelet 68±17 bin, tüm olgularda periferik yaymada hemoliz, protrombin zamanı 10.3±3 sn, fibrinojen 135±28 mg/dL ve idrarda proteinüri 5.8±0.6 g/24 saat olarak bulundu. Olgularımızın 4 ü (%20) primipar, 16 sı (%80) multipardı, 2si (%10) Class I, 18i (%90) Class II komplet tipik HELLP olgusu idi. Düzenli antenatal takibe gelme çok az olup (1.9±0.5 puan) idi. Olgularımızın 14ü (%70) vajinal yoldan dogurtulurken, 6sı (%30) sezaryan ile doğurtuldu, 6 sında (%30) hastaneye başvurduklarında fetus ölü idi. Postpartum bebeklerin 4ü (%20) respiratuar distres sendromu nedeniyle öldü. Perinatal mortalite %50, maternal mortalite yok idi. Olgularımızdan 2sinde (%10) plasenta dekolmanı, 6sında (%30) akut böbrek yetmezliği, 4ünde (%20) yaygın damar içi pıhtılaşma, 3ünde (%15) postoperatif ciltaltı hematomu, 2sinde (%10) erişkin respiratuar distres sendromu gelişti.Sonuç: HELLP sendromu yüksek maternal ve perinatal komplikasyon ile seyreden bir sendromdur. Genellikle şiddetli preeklampsi olgularında görülen bu sendromun erken tanı ve tedavisi her obstetrisyenin en önemli görevi olmalıdır. Düzenli antenatal takip ile, HELLP sendromunun dikkatle araştırılarak erken tanı, hızla tedavisi ve operatif olgularda rutin cilt altı dren önerilebilir.
ABSTRACT
Objective: The assessment of clinical, biochemical features with the maternal and fetal complications in the pre-eclampsy cases with complete typical HELLP syndrome in our department.Setting: Department of Obstetrics and Gynecology at Firat University, School of Medicine, Elazig/TurkeyMaterial and Method: Complete typical HELLP Syndrome cases were analysed prospectively who were pooled out of 150 pre-eclampsy patients admitted between September 1997-September 1999. All cases were assessed in terms of clinical, laboratory parameters, as well as fetal and maternal complications. Relative risk to develop HELLP syndrome in severe pre-eclampsy cases was calculated.Results: Forty cases out of 150 (26.6%) were diagnosed as severe pre-eclampsy, of which 20 cases were found to have complete typical HELLP Syndrome (13.3%), whereas 110 cases (73.4%) were evaluated as pre-eclampsy of mild degree. HELLP Syndrome development risk in severe pre-eclampsy cases was calculated as RR=55. The values of significant biochemical parameters (mean±SD) were documented as SGPT 379±249 IU/L, SGOT 724±689 IU/L, LDH 2109±778 IU/L; with the hematological parameters as platelet count 68±17.103, protrombin time 10.3±3 s, fibrinogen 135±28 mg/dL and urine protein 5.8±0.6 g/day together with the hemolysis demonstrated in all peripheral smear. Four cases (20%) were primipar and 16 (80%) multipar; of which 2 of them (10%) were of Class I, whereas 18 (90%) of Class II complete HELLP Syndrome. Regular antenatal examination was accomplished in very little number of patients.(1.9±0.5 points). Fourteen cases (70%) underwent vaginal delivery and 6 (30%) Cesarean section, another 6 (30%) were with in utero mort fetal at the time of admittance. Four babies (20%) died of respiratory distress in the postpartum period. Perinatal mortality was found as 50%, but maternal mortality was absent. Two cases (10%) developed abruptio placentae, 6 (30%) acute renal insufficiency, 4 (20%) disseminated intravascular coagulation, 3 (15%) postoperative subcutaneous hematoma, and 2 (10%) adult respiratory distress syndrome.Conclusion: HELLP Syndrome is a pathology associated with high incidence of maternal and perinatal complications. Attention must be paid by every obstetrician to the early diagnosis and the treatment of this syndrome which is mainly encountered in severe pre-eclampsy cases. By means of regular antenatal observation, early diagnosis and prompt treatment of HELLP Syndrome is possible; besides routine subcutaneous drain placement can be proposed in case of operated cases.
