ÖZET
Amaç: Prospektif randomize çalışma ile gerçek stress inkontinansta propylen mesh ve endotacker kullanımı ile yapılan laparoskopik Burch procedürünü alternatif prosedürlerle karşılaştırmak. Çalışmanın Yapıldığı Yer: Ankara Eğitim ve Araştırma Hastanesi Kadın Hastalıkları ve Doğum Kliniği. Materyel ve Metod: Şubat 1996 ve Mayıs 1998 tarihleri arasında klinik ve laboratuvar testleri ile gerçek stres inkontinans tanısı konan toplam 23 hastaya laparoskopik extraperitoneal Burch prosedürü uygulandı. Bu operasyonda balon distansiyon sistemi endotacker ve polypropylen mesh kullanılarak endopelvik fasia Cooper ligamana asıldı. Bulgular: 21 hastamızda iyileşme (%91.3), 1 hastamızda rekürrens (%4.3), 1 hastamızda intraoperatif mesane yaralanması gelişti (%4.3). Sonuç: Kısa operasyon süresi,operasyon alanının daha iyi görülebilmesi, minimal kanama,perioperatif komplikasyon azlığı, hastanede kalış süresi kısalığı, günlük aktivitelere çabuk dönüş ve yüksek başarı oranı gibi avantajlar sunan bu operasyonun diğer inkontinans operasyonlarının çok güçlü bir alternatifi olduğunu düşünmekteyiz. Ancak yinede tam bir fikir sahibi olabilmek için uzun süreli prospektif randomize çalışmalara ihtiyaç vardır.
ABSTRACT
Objective: The aim this prospective randomized study was to compare the laparoscopic Burch procedure using propylen mesh and endotacker for the treatment of genuine stress incontinence by clinical symptoms and laboratory findings and the outcomes. Institution: Ankara Education and Research Hospital, Department of Obstetrics and Gynecology. Materials and Methods: Extraperitoneal laparoscopic Burch procedure was performed on 23 patients with genuine stress incontinence. In this procedure the endopelvic fascia was hung on to the Coopers ligament by using balloon distention system and propylen mesh. Findings: 21 patients (91.3%) were succesfully treated. Recurrence was observed in 1 patient(4.3%) and intraoperative vesicle injury in 1 patient (4.3%). Conclusion: We believe this procedure to be a good alternative to other urinary incontinence operations because of several advantages; such as short operation time, better exposure, less perioperative complications, short hospitalization, faster return to the daily life and high rate of success.But we still need long term prospective randomized studies for a definite decision.
