ÖZET
İki ektopik gebelik vakası laparoskopik teknik kullanılarak tedavi edildi. Tübün antimezenterik kısmından unipolar elektrokoter yardımıyla linear salpingotomi yapıldı ve gebelik materyali suprapubik bölgeden açılan ikinci delik yoluyla çıkarıldı. Bipolar elektrokoter kullanılarak kanama kontrolü sağlandıktan sonra batın ringer laktat ile yıkandı. Postoperatif olarak b-HCG ile izlenen hastalarda perioperatif ya da geç dönemde komplikasyon olmadı. Bir hastada postoperatif HSG yapıldı ve tüplerin bilateral olarak açık olduğu gösterildi. Bu hastada postoperatif 6. ve 12. aylarda iki istenmeyen gebelik gözlendi. Diğer hasta ise 2. aydan sonra kontrollere gelmedi. Bu çalışmada laparoskopinin belirli koşulları taşıyan tubal gebeliklerin tedavisinde laparotomiye alternatif bir metod olduğu sonucuna varıldı.
ABSTRACT
Two cases with ectopic pregnancy were managed by laparoscopic techniques. A linear salpingotomy was carried out on the antimesenteric side of the fallopian tube using an unipolar electrocautery and the products of conception were removed through a second puncture. The abdomen was rinsed with Ringer's lactate and bleeding was controlled by bipolar electrocautery. Patients were followed weekly b-HCG measurements until normal titers achieved. There were no perioperative or delayed postoperative complications. In one patients who had a HSG 4 months postoperatively bilateral tubal patency was demonstrated. This patients had two uplanned pregnancies. The other patients lost to follow-up after two months. We concluded that the laparoscopy may be an alternative to laparotomy for the treatment of tubal pregnancy in certain cases.
