ÖZET
Adneksiyal torsion, nadir görülmesine rağmen, jinekolojik acillerden bir tanesidir ve akut alt abdominal ağrı varlığında akla gelmesi gereken tanılardan biri olmalıdır. Adneksiyal torsiyon adolesanlar ve genç kadınların reprodüktif geleceğini etkileyebilen önemli bir klinik antitedir. Adneksiyal torsiyon neoplastik overlerde gözlenebileceği gibi, hiperstimülasyon sonucunda da ortaya çıkabilir. Adneksi oluşturan yapılar olan tüpler ve overler torsiyone olabilir. Torsiyonun klinik bulgularının nonspesifik olması tanı ve tedavinin gecikmesine neden olabilmektedir. Klasik prezentasyon, peritonit ve adneksiyal kitle ile birlikte gözlenen akut başlangıçlı abdominal ağrıdır. Fakat ne yazık ki, hastaların çoğundaki mevcut bulgular tanı koyduracak kadar spesifik değildir. Adneksiyal torsiyonun, pelvik konjesyon neticesinde premenstrüel dönemde ya da ovulasyon sırasında oluşması beklenirken, menstrüel siklusun evresi ve torsiyon arasında henüz bir ilişki bulunamamıştır. Peritonit ve ölümle sonuçlanabileceği için adneksiyal torsiyonun tedavisi acilen gerçekleştirilmelidir. Geçmişte, detorsiyone edilen adneksin tromboemboli ve enfeksiyona yol açabileceğini düşünen bazı otörler adneksin tamamen çıkarılmasını önermekteydi. Fakat günümüzde adneksin detorsiyone edilerek reperfüzyonun ve viabilitenin izlenmesinin güvenli bir yaklaşım olduğu görüşü hakimdir. Buna rağmen, cerrahi girişimdeki gecikmeler adnekslerin nekrozu neticesinde tüp, over ya da her ikisinin birden kaybı ile sonuçlanabilmektedir. Özellikle yardımcı üreme tekniklerinin yaygın kullanımı ile kontrollü ovaryan hiperstimülasyon sonucunda oluşan ovaryen torsiyon, akut abdominal ağrı nedenlerinden biri olarak akla gelmesi önemli olan komplikasyonlardan biri olmuştur. Bu olgularda laparoskopi, gecikmeden uygulanması gereken altın standart yaklaşımdır. Bizim amacımız da bu konudaki literatürü gözden geçirerek, klinik kullanımdaki yerini eldeki veriler ışığında tartışmaktır.
ABSTRACT
Adnexal torsion is one of the gynecological surgical emergencies which, despite its infrequent occurrence should always be highly ranked in the index suspicion whenever dealing with acute lower abdominal pain. Adnexal torsion may have a devastating effect on the future reproductive performance of adolescents and young women. It may occur in neoplastic ovaries or as a consequence of hyperstimulation. Any portion of the adnexa, either the tube or the ovary may undergo torsion. The clinical findings of torsion are usually nonspecific, for this reason, delays in diagnosis and surgical intervention may be substantial. The classic presentation is the acute onset of abdominal pain with clinical evidence of peritonitis and an adnexal mass. But unfortunately, the presenting findings in most patients are nonspecific and unimpressive. Torsion is more likely to occur during ovulation or as premenstrual event associated with increased pelvic congestion; but no correlation has been found between the phase of the cycle and the onset of the symptoms. Treatment of adnexal torsion is considered an emergency because peritonitis and death can result. Historically, the adnexa usually were removed because some authors suggested that the unwinding the adnexa could increase the risk of tromboembolism and infection. There is growing evidence that unwinding the involved adnexa to observe for tissue reperfusion and viability is safe. Nevertheless, a significant delay in surgical intervention may result in irreversible necrosis requiring removal of the tube, ovary, or both. Especially in controlled hiperstimulation of assisted reproductive techniques, adnexal torsion is one of the clinical emergencies representing with acute abdominal pain. Laparoscopy is the gold standart for these cases which must be performed without delay. Our aim is to review the literature on this comorbidity .
