Maternal And Fetal Outcomes Of Continuous Epidural Analgesia In Nulliparous Pregnanat Women

ÖZET
Amaç: Obstetride doğum ağrılarının kontrolünde en çok kullanılan yöntemlerden biri epidural analjezidir. Bu çalışmada sürekli epidural analjezinin anne ve yenidoğan üzerindeki etkileri incelendi. Gereç ve Yöntemler: Çalışmaya 37-41. gebelik haftaları arasındaki, risk faktörü bulunmayan, 30 epidural, 30 kontrol olgusundan oluşan 60 nullipar gebe dahil edildi. Epidural kateterden içerisinde 50 mikrogram fentanil ve %0.125'lik 12.5 mg bupivakain bulunan 10 ml solüsyon bolus yapıldı. Sonra 10 ml'sinde 15 mikrogram fentanil ve %0.125'lik 5 mg bupivakain içeren 100 ml solüsyon hazırlandı. 10 ml/sa olacak şekilde sürekli infüzyona başlandı. Gruplar yaş, boy, vücut ağırlığı, sistolik ve diastolik arter basınçları, nabız, doğum evreleri, doğum şekli, yenidoğanın ağırlığı, 1. ve 5. dakika APGAR skorları, umbilikal kord kan gazları ve doğum süresince oluşan yan etkiler açısından karşılaştırıldı. Bulgular: Her iki grup arasında yaş, gebelik haftası, boy, vücut ağırlığı, nabız, sistolik ve diastolik basınçlar açısından anlamlı bir farklılık tespit edilmedi. Epidural analjezi uygulanan grupta doğumun 1. evresi istatistiksel olarak anlamlı şekilde kısalırken 2. evresi kontrol grubuna göre daha uzun bulundu (p> 0.05) . Epidural analjezi yapılan grupta oksitosin ile indüksiyon ihtiyacı kontrol grubuna göre daha yüksek oranda görüldü. Gruplar arasında doğum şekilleri, APGAR skorları, yenidoğanın ağırlığı, kord pH, pO2, pCO2 ve HCO3 değerleri açısından anlamlı fark olmadığı tespit edildi. Epidural analjezi grubunda en sık rastlanan yan etkiler bulantı, kusma ve hipotansiyon idi. Sonuç: Sürekli epidural analjezi anne ve yenidoğan açısından ciddi bir yan bir etki oluşturmamaktadır. Bu nedenle doğum ağrılarının giderilmesinde kullanılabilir.
ABSTRACT
Objective: Most frequent method used to control of pain of labor is epidural analgesia. In this study maternal and fetal effects of epidural analgesia were evaluated. Material and Methods: Sixty nulliparous prepnant women between 37-41 gestational weeks without any risk factors were divided into 2 groups: Thirty subjects as epidural and 30 subjects as control group. Ten ml 0.125% 12.5 mg bupivacaine solution containing 50 mcg fentanyl was given with epidural catheter as bolous. Then 100 ml solution was prepared as containing 0.125% 5 mg bupivacaine and 15 mcg fentanyl in 10 ml solution. This was given by 10 ml/hr continuous infusion. Two groups were compared in terms of age, height, weight, arterial blood pressure, heart rate, stages of labor, type of labor, birthweight, APGAR scores at 1 and 5 minutes, umbilical cord blood gases and adverse-effects occured during labor. Results: There were no statistically significant differences between the 2 groups in terms of age, height, weight, pregnancy week, heart rate, arterial blood pressure. First stage of labor was significantly shorter in the epidural group. Second stage was longer in the epidural group than control group, but this was not statistically significant. Oxytocine was used for the induction of labor more frequently in the epidural analgesia group. There were no statistically significant differences between the two groups in terms of type of labor, APGAR scores, birthweight, cord pH, pO2 and pCO2 tensions and HCO3 levels. Most frequent adverse effects in the epidural analgesia group were nausea-vomiting and hypotension. Conclusion: Continuous epidural anelgesia has no maternal and fetal severe adverse effects. It can be used for prevention of pain of labor.
