Monitoring Of Hormone Replacement Therapy With Kupperman Index, Plasma Estradiol Level And Hypoestrogenemic Index

ÖZET
Amaç: Doğal ve cerrahi yol ile menopoza giren hastaların hormon replasman tedavisine verdikleri cevabın monitorizasyonunda Kupperman indeksi, hipoöstrojenemik indeks ve plazma östradiol (E2) düzeyinin etkinliğini araştırmak. Materyel ve Metod: Doğal ve cerrahi yoldan menopoza giren 247 hasta çalışmamızın materyalini oluşturdu. Tüm hastaların tedavi öncesi biyokimyasal parametreleri, plazma E2 düzeyleri ve subjektif yakınmaları Kupperman indeksi ve hipoöstrojenemik indeks ile sorgulanarak tespit edildi. Değişik hormon replasman tedavi protokolleri uygulanan hastaların 1., 2., 3. aylık kontrollerinde bu parametreler tekrarlanarak tedavinin etkinliği araştırıldı. Üçüncü ayda istenilen plazma E2 düzeyi ve diğer subjektif değerlendirme skorlarında istenilen düzelmeler sağlanamayan hastalarda ilaç dozu ayarlanılarak üç ay daha takip edildiler. Bulgular: Tüm hastaların tedavi öncesi Kupperman indeks ortalaması ve standart sapması 22,43±9,2, hipoöstrojenemik indeks ortalaması ve standart sapmasi 5,91± 4, plazma E2 düzeyi ortalaması ve standart sapması 19,8 ± 24,3 pg/ml olarak bulundu. Tedavi sonrasi, birinci ay plazma E2 düzeyi ortalaması ve standart sapması 38,32±74, Kupperman indeks ortalaması ve standart sapması 8.25±6.8, hipoöstrojenemik indeks ortalaması ve standart sapması 4,57±4,10 olarak bulundu. Tedavi öncesi ve sonrasi plazma E2 düzeyi, Kupperman indeks ve hipoöstrojenemik indeks ortalamalarında istatiksel olarak anlamlı artışlar olmakla birlikte (p<0,01) standart sapmaların istatiksel olarak ileri derecede farklı olması değişik tedavi protokollerine hastaların verdiği cevabın hatta aynı tedavi protokollerine hastaların verdiği cevabın değiştiğini göstermektedir (ANOVA p<0.01). Sonuç: İster değisik hormon replasman tedavi protokolleri uygulansın, isterse aynı hormon replasman tedavi protokolü birçok hastaya uygulansın tedavinin etkinliğini tespit edebilmek için hastaların tedavi kişiselleştirilinceye kadar (ortalama üç ay) Kupperman indeksi, plazma E2 düzeyi ve hipoöstrojenemik indeks ile monitorizasyonunun ve buna göre tedavi dozlarının düzenlenmesi gerektiği kanısındayız.
ABSTRACT
Objective: The aim of this study was to evaluate Kupperman Index, hypoestrogenic index and plasma estradiol (E2) level in monitoring hormonal replacement therapy responses in - either spontaneous or surgically induced- menopausal women. Material and Methods: The total of 247 women in menopause participated in our study. On all women biochemical parameters, plasma E2 level, Kupperman - , and hypoestrogenic indexes were evaluated preceding treatment. Different standardized hormonal replacement therapies were implied on the women. After therapy, all parameters were reevaluated and registered at the end of the 1st, 2nd, 3rd months . At the end of the third month subjects in whom we couldnt reach therapeutic plasma estradiol levels and desired subjective scors, we months) of hormone levels and those indexes. It has been proven by our study, that a standardization to one average dosage appropriate for all patients is not possible.rearranged dosages and they are followed up for another three month. After the first month of treatment the results with mean and standart deviations changed as follows: 8.25±6.8 for Kupperman index; 4.57±4.1 for hypoestrogenic index and 38.32±74 pg/ml for plasma estradiol level. Even though there is a statistic difference between all parameters before and after treatment, there is a wide range of response to treatment as the standard deviation is quite high. Especially the plasma estradiol level shows a significant difference in comparison (ANOVA p<0.01). Conclusion: Even by using standardized hormonal-replacement therapy (HRT) we suggest in any case that in treatment an aim is made to reach individual therapeutic dose in each patient by monthly checks (approximately 3
