Peri-urethral Macroplastique (Silicone Microimplant) Injection For The Treatment Of Intrinsic Sphincter Defficiency: A Case Report

ÖZET
Giriş: İntrinsik sfinkter yetmezliği üriner inkontiansın önemli sebeplerinden biridir. Tedavisinde subürethral sling prosedürü, artifisiyel sfinkter ve periürethral enjeksiyon yöntemleri uygulanmaktadır. En kolay ve en sık uygulanan yöntem olan periürethral enjeksiyonların da değişik tipleri bulunmaktadır. Macroplastique (polidimetilsiloxan) enjeksiyonu da bunlardan biridir. Biz de daha önce inkontinans cerrahisi geçiren ve nüks stres inkontinans tanısı koyduğumuz bir hastamızda Macroplastique implants enjeksiyonu uygulayarak bu tedavi yöntemini değerlendirmeyi amaçladık. Olgu Sunumu: 52 yaşında, paritesi 4 olan hastanın özgeçmişinde 1995 yılında geçirilmiş vajinal histerektomi ve colporafi anterior-posterior + Kely plikasyonu mevcut. İdrar kaçırma şikayeti ile başvuran hastanın ürodinamik çalışmaları intrinsik sfinkter yetmezliği ile uyumlu bulundu. Teknik: Silikon implant olarak solid yapıda steril polidimetilsiloxsan kullanıldı (Macroplastique, Uroplasty, Hollanda). Genel anestezi altında litotomi pozisyonunda sistoskopi eşliğinde, endoskopik iğne yardımıyla, üretranın submukozal bölgesine saat 2,6 ve10 hizalarına macroplastique implants enjekte edildi. Tartışma: Intrinsek sfinkter yetmezliğinde daha çok üretral kompresyon oluşturabilecek subüretral sling prosedürü, artifisiel sfinkter ve periüretral enjeksiyon gibi prosedürlere ihtiyaç duyulmaktadır. Bu prosedürlerden ilk ikisinin teknik olarak zor ve komplikasyonlarının fazla olmasıperiürethral enjeksiyonları ön plana çıkarmıştır. Periürethral Macroplastique enjeksiyonları ile başarılı sonuçlar bildirilmiş ve bu partiküllerin migrasyon göstermediği saptanmıştır. Bizim vakamızda da 1 yıl sonunda stres inkontinans halinin tamamen ortadan kaybolduğu izlenmiştir. Yorum: Yüksek başarı oranları bildirilmekle birlikte silikon ile uzun dönem sonuçları olmadığı için bu prosedürün ancak daha önce başarısız inkontinans cerrahisi geçiren ve/veya major cerrahiyi kaldıramayacak intrinsek sfinkter yetmezliği olan hastalarda kullanılabileceğini düşünmekteyiz.
ABSTRACT
Introduction: Suburethral sling procedures , artificial urinary sphincter application and periurethral injections are commonly used treatment modalities for intrinsic sphincter deficiency which is one of the major cause of urinary incontinence. Macroplastique (polidimetilsiloxan) injection is one of the injection types. We applied this procedure to a patient with recurrent stress incontinence who had a previous Kelly plication in our department. Case: A 52-years-old woman with gravida 4, parite 4 was admitted to our department with the complaint of urinary incontinence. She had had an operation in 1995 composed of vaginal hysterectomy, colporraphy anterior-posterior and Kelly plication. Urodynamic evaluations revealed intrinsic sphincter defficiency. Technique: Sterile polidimetilsiloxsane (Macroplastique, Uroplasty, Netherland) was used as silicone implant. Under general anesthesia and at lithohomy position, Macroplastique was injected at 2, 6, 10 oclock position around the suburethral area through endoscopic needle instrument under the cystoscopic vision. Results and Conclusions: There are different modalities for treatment of intrinsic sphincter defficiency. Periurethral injections are the most easily applied and the least complicated procedure of them. Macroplastique injection is a succesfull procedure without the risk of silicone migration which proved with experimantal studies. In our case, we also did not observed stress incontinence at the end of 1 year follow-up period postoperatively. Absence of information on the long-term safety of silicone might have to limit its wide use. It may be thought as a suitable procedure for selected cases with intrinsiin failure before and/or has poor condition for major and extended surgery.
