ÖZET
Plasental site trofoblastik tümör (PSTT) intermediate trofoblastik hücrelerin oluşturduğu nadir bir gestasyonel trofoblastik hastalık (GTH)' tır. Çoğu olguda benign seyirli olmakla birlikte %15-20 olguda agresif davranış gösterebilir. Anormal uterin kanama yakınması ile başvuran 32 yaşında kadın hastaya, yapılan ultrasonografik incelemede uterin kavitede kitle görünümü saptanması ve serum ?-hCG düzeyinin 118 mIU/ml olması nedeniyle ile tanısal amaçlı küretaj uygulanmıştır. Histopatolojik incelemede miyometriyuma invazyon gösteren intermediate karakterde trofoblastik hücre proliferasyonu görülmesi ve koryonik villus yapısı izlenmemesi nedeniyle bulgular öncelikle PSTT ile uyumlu olarak değerlendirilmiştir. Küretaj sonrası ?-hCG seviyesinde azalma olmayan hastaya TAH+ sağ salpingooferektomi operasyonu uygulanmıştır. Histopatolojik incelemede neoplastik karakterde trofoblastik hücrelerin miyomeriumu, serozayı, sağ tuba ve over dokusunu infiltre ettiği izlenmiştir. İmmunhistokimyasal incelemede tümör hücrelerinde hPL ile yaygın, hCG ile fokal reaksiyon izlenmiştir. Operasyon sonrası 3. ayda ?-hCG düzeyi 0.9 mIU/ml olarak saptanmıştır. PSTT olgularında operasyon öncesi kesin tanı verilmesi oldukça zordur. Histopatolojik ayırıcı tanıda immunhistokimyasal incelemeler önemli katkı sağlamaktadır. Olgumuz nadir görülen bir hastalık olması, uterus dışına yayılım göstermesi ve küretaj materyalinde tanı alması nedeniyle literatür bilgileri eşliğinde sunulmuştur.
ABSTRACT
Placental site trophoblastic tumour (PSTT) is a rare gestational trophoblastic disease (GTD) consisting of intermediate trophoblastic cells. Despite its having a benign course in most cases, in 15-20% of the cases it might display aggressive behaviour. Diagnostic curettage was performed in a 32 year old woman admitted with the complaint of abnormal uterine bleeding after detection of a mass image in the uterine cavity in the ultrasonographic examination and a serum ?-hCG level of 118 mIU/ml. In the histopathological examination, the findings have been interpreted to be primarily compatible with PSTT because of the detection of trophoblastic cell proliferation of intermediate characteristic that invades the myometrial tissue and absence of a chorionic villus structure. TAH+ right salphingo oopheroctomy operation was performed on the patient in whom there was no reduction in the post-curettage ?-hCG level. In the histopathological examination trophoblastic cells of neoplastic characteristics were observed to infiltrate all layers of the myometrium, serosa, right tuba and the ovarian tissue. In the immunohistochemical examination, a widespread reaction with hPL and a focal reaction with hCG was observed in the tumour cells. At 3 months after the operation, ?-hCG level was determined as 0.9 mIU/ml. In PSTT cases it is very difficult to make a definitive diagnosis before the operation. In the histopathological differential diagnosis immunohistochemical examinations make a significant contribution. Our case has been presented accompanied by literature information since it is a rare disease, spreads outside the uterus and is diagnosed with the curettage material.
