ÖZET
Amaç: Bu çalışmanın amacı postmenopozal kadınlarda kısa süreli tibolon replasman tedavisinin arterler üzerindeki etkilerini prospektif olarak renkli Doppler ultrasonografi ile incelemektir. Materyal ve Metod: Dicle Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalında 1 Ocak 2000 ile 31 Aralık 2000 tarihleri arasında postmenopozal şikayetleri nedeniyle polikliniğimize başvuran, menopoz süresi >12 ay ve daha önce hormon replasman tedavisi (HRT) almamış 52 olgu prospektif olarak incelendi. Takibe gelmeyen 11 ve intraabdominal hemoraji gelişen bir olgu çalışma dışında tutuldu, geri kalan 40 olgu çalışma kapsamına alındı. Tüm olguların ilk başvurularında renkli Doppler ultrasonografi ile femoral arter ve dallarında kan akım hızları ölçüldü. Olgular iki gruba ayrıldı, grup Ie (n=28) 2.5 mg tibolon oral verildi, ve grup IIye (n=12) hiçbir tedavi uygulanmadı. Olgular 12 hafta sonra kontrole çağrılıp tedavinin klinik etkinliği sorgulandı ve arteriyel kan akım hızları tekrar ölçüldü. Tüm ölçümler aynı kişi tarafından yapıldı. İstatistiksel analizlerde SPSS for Windows 10 programı, grupların karşılaştırılmasında Wilcoxon Rank Sum testi kullanıldı.Bulgular: Olguların ortalama yaşı 51.71±5.87, ortalama menopoz yaşı 49.50±3.71, ortalama menopoz süresi 2.65±2.09 yıl olarak bulundu. Tedavi öncesi ve sonrasında grup Ide ana femoral arterde Vmax (cm/sn) hızı 77.16±16.82den 69.85±9.90?a (p<0.05), derin femoral arterde 53.05±11.26dan 47.39±7.20ye (p<0.05), yüzeyel femoral arterde 76.46±12.09dan 75.73±9.99a (p>0.05), distal femoral arterde 54.43±16.46dan 48.50±9.56ya (p<0.05) düştüğü gözlendi. Kontrol grubunda ise ilk ve son ölçümler arasındaki fark anlamlı bulunmadı (p>0.05). Sonuç: Çalışmamızda 12 haftalık tibolon kullanılması sonucunda femoral arter ve dallarında renkli Doppler ultrasonografi incelemesi ile arteriyel kan akım hızlarının önemli ölçüde azaldığı gözlendi. Postmenopozal kadınlarda hormon replasman tedavisinin damarlar üzerindeki etkilerini izlemek için noninvazif bir yöntem olan renkli Doppler ultrasonografi kullanılmasının objektif bulguları göstermesi yönünden yararlı olduğu düşünüldü.
ABSTRACT
Objective: The aim of this study is to investigate prospectively with color Doppler ultrasound, the effects of short time tibolon treatment on the femoral artery blood flow velocities.Material and Methods: Fifty two women, who have not received any hormone replacement therapy (HRT), and are on postmenopausal period at least since >12 months were studied prospectively, between 1 January 2000 and 31 December 2000 at Dicle University Faculty of Medicine, Department of Obstetrics and Gynecology and Department of Radiodiagnostic. Twelve patients were excluded because of eleven were failed the following and one developed intraabdominal hemorrhage, the rest 40 patients were included in the study. The femoral artery blood flow velocity waveforms were measured with color Doppler ultrasound in all patients at the first visit. The patients were divided in two groups, and tibolon 2.5 mg was given orally to group I (n=28), and any medication was given to group II (n=12). All patients were invited 12 weeks later for the evaluation of the clinical drug effects, and femoral artery blood flow velocity waveforms were measured again. The same person carried out all measurements. The statistical analyses were performed using SPSS for Windows 10 program. Wilcoxon Rank Sum test was used for the comparison of the groups. Results: Mean ages of cases 51.71±5.87, menopause ages 49.50±3.71, and menopause period 2.65±2.09 were found. Before and after treatment the main femoral artery Vmax decreased from 77.16±16.82 to 69.85±9.90 (p<0.05), deep femoral artery Vmax from 53.05±11.26 to 47.39±7.20 (p<0.05), the superficial femoral artery Vmax from 76.46±12.09 to 75.73±9.99 (p>0.05), and distal femoral artery Vmax from 54.43±16.46 to 48.50±9.56 in the group I. In the first and the last visits, femoral artery Vmax measurements were not found significantly different in group II (p>0.05).Conclusions: The use of color Doppler ultrasound on the arterial blood flow velocity waveforms of femoral artery and its branches on postmenopausal women showed statistically significant decrease at the end of 12th week. Therefore, as a non invasive test, color Doppler ultrasound can be used to point out the objective effects of HRT on blood vessels in postmenopausal women
