Recurrent Abortions And Balanced Translocation 46, XX, t(3;7) (p21;q36) Carriers Mother And Daughter, Two Cases

ÖZET
Yapısal kromozomal yeni düzenlenmeler, genel populasyonda yaygın olarak gözlenmektedir. Dengeli translokasyon taşıyıcısı bireylerin, dengesiz kromozomlu çocuklara sahip olma riskleri vardır. Bu yüzden tekrarlayan düşüklere sahip çiftlerde translokasyon sıklığı, genel populasyondakinden daha yüksektir. Bu çalışmada OMÜ Tıp Fakültesi Kadın Doğum Anabilim Dalın'dan tekrarlayan düşük nedeniyle gönderilen, 20 yaşında anatomik olarak normal ve sistemik bir hastalığı olmayan, ilk trimestri 10, 8,12 ve 8 haftalık dört düşüğü olan ve yaşayan hiç çocuğu olmayan kadın olgu genetik incelemeye alınmıştır. Probandın aile öyküsünde 50 yaşında 13 düşüğü ve yaşayan 4 kızı 1 oğlu olan annesi ve mental retardasyonu olan 1 kız kardeşi olduğu kaydedilmiştir. GTG bantlama yöntemi ile yapılan analiz sonucunda proband ve annesinin 46,XX, t(3;7) (p21;q36) dengeli translokasyon taşıyıcısı oldukları saptanmıştır. Probandın zihinsel özürlü kızkardeşinden henüz sitogenetik analiz yapılamamıştır. Aile öyküsünde tekrarlayan düşüklerin ve mental retardasyonun olduğu olgumuzda, dengeli translokasyon taşıyıcılarının doğacak çocuklara verdikleri dengesiz kromozomlarla düşük ve anomalili doğum riskinin arttığı gözlenmiştir.
ABSTRACT
The structural chromosomal new rearrangements are common in general population. Even balanced translocation carriers could have risk for having children with unbalanced chromosomes. Therefore, translocation can be observed with a higher incidence in couples with a history of recurrent abortions then the general population. In this study, we investigated a 20 year old female patient, send from Department of Gynecology and Obstetrics, Faculty of Medicine, Ondokuz Mayıs University. She did not have an anatomic disorder or any systemic disease in her history. She had four abortions in the first trimester; at 10, 8, 12 and 8 weeks. She does not any alive children. The family history of proband revealed that her 50 years old mother had 13 abortions and four girls and one boy alive and her sister has mental retardation. Results of G-banding karyotype analysis, 46, XX, t(3;7) (p21;q36) balanced translocation was diagnosed in the specimens taken from her and her mother. However cytogenetic analysis of her sister suffering from mental retardation has not been accomplished yet. The recurrent abortions and mental retardation comes out from the family history in this case. Balanced translocation carriers could give unbalanced chromosomes to their new born child. Also this observed situation shows an increase in the risk of abortion and anomalies of the new born.
