Appendiceal Mucocele Presenting as a Right Adnexal Mass: Case Report
Sağ Adneksiyal Kitle Kliniği ile Prezente Olan Apendiks Mukoseli

ÖZET
Apendiks mukoseli, nadir görülen ve sağ adneksiyal kitleyi taklit edebilen bir antitedir. Apendiks lümeninde mukus birikimine ikincil olarak apendiksin dilatasyonudur. Bu çalışmada, postmenopozal apendiks mukoseli tanısı alan bir olgu sunulmuştur. Kliniğimize sağ kasık ağrısı nedeni ile başvuran 54 yaşındaki postmenopozal olgunun pelvik ve ultrasonografik muayenesi sonrası sağ adneksiyal alandan kaynaklanan 3x5 cm boyutunda kitle tespit edildi. Serum CA 125 düzeyleri normaldi (CA 125:14), abdominal tomografi de aynı bulguları destekledi. Sağ adneksiyal kitle ön tanısı ile laparotomi uygulanan olguda, kitlenin apendiks kaynaklı olduğu görüldü ve apendektomi yapıldı. Histopatolojik olarak apendiks mukoseli tanısı konuldu. Apendiks mukoseli tanısında ultrasonografi, bilgisayarlı tomografi, manyetik rezonans görüntüleme faydalıdır. Ancak preoperatif tanısı zordur ve genellikle rastlantısal saptanır. Ender görülmekle birlikte, sağ adneksiyal kitlelerin ayırıcı tanısında apendiks mukoseli de akla gelmelidir.
ABSTRACT
Mucocele of vermiform appendix is a rare entity that may mimic a righ-sided adnexal mass. It is formed by cystic dilatation, abnormal mucinous secretion of the appendiceal lumen. Here we aimed to report case of appendiceal mucocele. 54-year-old postmenopausal woman was admitted with a right lower abdominal pain to our clinic. Pelvic and ultrasonographic examinations revealed a mass arising from adnexal area on the right side extending for about 3x5 cm. The serum CA 125 level was within normal limit (CA 125:14). Computerize tomography identified same diagnosis. During the laparotomy the right adnexal mass was appendiceal in origin and appendectomy was performed. Histology confirmed a diagnosis of appendiceal mucocele. Various radiological tools including ultrasound, computer tomography scan, magnetic resonance imaging may be useful for the diagnosis of appendiceal mucocele. But pre-operative diagnosis is difficult and it is diagnosed incidentally. Although appendiceal mucocele is a rare entity it should be considered in the differential diagnosis of adnexal masses.
