ÖZET
Selektif östrojen reseptör modülatörleri (SERM) farklı dokulardaki östrojen reseptörlerinin agonistik/antagonistik profilini düzenleyebilen bir grup bileşikten oluşmaktadır. Selektif östrojen reseptör modülatörleri östrojen replasman tedavisinin istenmeyen etkilerini ortadan kaldırmak ya da azaltmak amacı ile kullanıma sunulmuştur. Meme ve endometriumda negatif, kemik, kardiyovasküler sistem ve vajende pozitif etki gösteren SERM ideal hormon replasman tedavisi ilacı olarak kullanılabilir. Günümüzde SERM profiline sahip birçok ilaç klinikte kullanılmaktadır. Tamoxifen ve toremifene meme kanserinde kullanılmaktadır. Tamoxifen kemik mineral yoğunluğu ve kan lipidlerinde olumlu etkiye sahiptir. Her iki ilaç endometriumu uyarmaktadır. Raloxifene bu etkiyi göstermeden kemik mineral yoğunluğunu arttırır. Östrojene benzer olarak SERM ler venöz tromboemboli riskini arttırırlar. Bu makalede SERM lerin etki mekanizmaları kısaca gözden geçirilerek klinik kullanımdaki yerleri eldeki veriler ışığında tartışılacaktır.
ABSTRACT
Selective estrogen receptor modulators (SERMs) constitute a group of compounds which have different agonistic and antagonistic affect on estrogen receptors of distinct tissues. Capability of different estrogenic or antiestrogenic affect on different tissues enables their use in the clinical practice. They have negative affect on mammary and endometrial tissue while exhibiting positive affect on bone, cardiovascular system and vagina. This selective affect forms their basis for different pathologies. Today we use different compounds in the profile of SERM in clinical practice. Tamoxifen and toremifene have their potential in the treatment of breast cancer. Tamoxifen has positive affect on bone and lipid profile. Both compounds have stimulatory affect on endometrium. Raloxifene is increasing bone mineral density without affecting endometrium. All of the compounds increase the risk of venous tromboembolism as estrogens. In this review we summarized the receptor SERM interactions and type and use of these compounds in clinical practice.
