ÖZET
Sezaryen skar gebelik (SSG)'leri, ektopik gebeliklerin ciddi komplikasyonlara sebep olabilen en nadir formudur. SSG insidansı son 10 yılda artan sezaryen oranlarıyla birlikte dünya çapında yükselmeye başlamıştır. Erken ve doğru teşhis ile terminasyon tedavi yönetiminde en önemli noktaları oluşturmaktadır. SSG klinik yönetiminde masif kan kaybının önlenmesi ve fertilitenin devamı için uterusun korunması temel amaçtır. Standart tedavi modalitesi hâlâ bilinmemektedir. Tedavi seçenekleri içinde dilatasyon ve küretaj, lokal veya sistemik metotreksat uygulaması, laparatomi, laparoskopi veya histeroskopi ile trofoblastik dokunun çıkarılması ve son zamanlarda denenen uterin arter embolizasyonu bulunmaktadır. Bu olgu sunumunda, sezaryen skar gebeliği olan ve dilatasyon ve küretaj ile tedavi edilen bir olguyu paylaştık.
ABSTRACT
Cesarean scar pregnancy (CSP) is the rarest form of ectopic pregnancies that can cause serious complications. The incidence of CSP has risen during the last decade because of rising cesarean section rates worldwide. An early and accurate diagnosis and termination constitute main points of the management. Clinical management of CSP should be the prevention of massive blood loss and the conservation of the uterus to maintain further fertility. Standard treatment of choice is still unknown. Many therapeutic options are available such as dilatation and curettage, local or systemically administered methotrexate, laparatomy, excision of trophoblastic tissues using either laparoscopy or hysteroscopy and uterine artery embolization. In this report, we present a patient with cesarean scar pregnancy who was successfully treated with dilatation and curettage.
