Laparoscopic Hysterectomy And Laparoscopic Pelvic Lymph Nodesampling In Treatment Of Stage I Endometrium Cancer: Report Of Two Cases

ÖZET
Amaç: Endometrium kanseri, gelişmiş ülkelerde en sık görülen kadın genital sistem kanseridir. Hastalığın tedavisi için erken tanı konması kadar doğru cerrahi evreleme yapılması da önemlidir. Laparoskopik cerrahide son 20 yıl içinde olan ilerlemeler ile laparoskopi asiste cerrahi evreleme (Laparoscopy Assisted Surgical Staging LASS) klasik yaklaşıma alternatif bir uygulamadır. Bu bağlamda kliniğimizde ilk kez laparoskopi asiste cerrahi evreleme uygulanan iki olguyu sunmayı amaçladık. Çalışmanın Yapıldığı Yer: Gazi Üniversitesi Kadın Hastalıkları ve Doğum Anabilim Dalı. Materyel ve Metod: Grade II endometrioid tipte adenokarsinom tanısı alan biri 53 diğeri 54 yaşında iki hastadan ilkine laparoskopi asiste vajinal histerektomi, bilateral salpingooferektomi, pelvik lenf nodu örneklemesi ikincisine total laparoskopik histerektomi, bilateral salpingooferektomi, pelvik lenf nodu örneklemesi yapılmıştır. Bulgular: Her iki hastanın da operasyonunu takiben postoperatif erken veya geç komplikasyon olmamıştır. Hastaların her ikisinde de myometrial invazyon 1/2nin altında idi. Operasyon süresi ilk olguda 120 dakika ikinci olguda 150 dakika olup operasyonda çıkartılan lenf nodu sayısı ilk vakada 12 adet, ikinci vakada 22 adetdi. Kan kaybı ilk olguda 300 cc ikinci olguda 500 cc olup her iki hasta da operasyon sonrası evresi evre IB olarak kabul edildi. İlk hastanın geçen 6 aylık süredeki takiplerinde sorunu olmamıştır ve halen izlemi sürmektedir. Sonuç: Bu sonuçlar itibari ile verilerimiz literatürde yayınlanan seriler ile paralellik göstermektedir. Gerek laparoskopi asiste vajinal histerektomi, gerekse total laparoskopik histerektomi ve bilateral salpingooferektomi, pelvik lenf nodu örneklemesi endometrial adenokanserli hastaların cerrahi evrelemesi ve tedavisinde tecrübeli laparoskopik cerrahların varlığında abdominal yolla gerçekleştirilen operasyonlar kadar güvenilir ve başarılı bir yaklaşımdır.
ABSTRACT
Objective: Endometrial cancer is the most common malignant disease of female genital system in developed countries. Besides early diagnosis, careful surgical staging is important for the cure of the disease. Laparoscopy assisted surgical staging is an alternative surgical approach due to developments in laparoscopic surgery for the last twenty years. In this article we presented two cases which were staged by laparoscopically assisted technique. Institution: Gazi University Faculty of Medicine, Department of Obstetrics and Gynecology. Materials and Method: We performed the laparoscopy assisted vaginal hysterectomy, and bilateral salpingoophorectomy, cytology and pelvic lymph node sampling to a 53 years old woman who has a grade II endometrial adenocarcinoma and laparoscopic total hysterectomy, bilateral salpingoopherectomy, cytology and pelvic lymph node sampling to a 54 years old woman who has a grade II endometrial adenocarcinoma. Results: There were no complications during early and late postoperative periods of the two patients. Myometrial invasion was below 1/2 thickness of myometrium in these case. There were no perioperative and postoperative complications in these case and operation time was approximately 120 minutes 150 minutes respectively. Number of harvested pelvic lymph nodes were 12 for first one and 22 second one. Blood loss were 300 cc and 500 cc respectively. These two cases were accepted as stage IB. Postoperative 6 months of the patient one is reached and there occurred no problem yet. Conclusion: Our operation results were similar to other reports. Laparoscopy assisted vaginal or total hysterectomy, and bilateral salpingoophorectomy, cytology and pelvic lymph node sampling are as reliable and succesful as traditional approach for surgical staging and treatment of endometrial cancer, if conducted by experienced surgeons.
