Combined Surgical Treatment Of Benign Gynecological Problems With Insitu VaGinal Wall Sling Procedure For Stress Urinary Incontinence : Three Years Experience

ÖZET
Amaç: Stress inkontinans, sistosel, myoma uteri, adneksiyal kitle ve benzeri jinekolojik sorunları birarada olan hastalara yapılan insitu vajinal wall sling operasyonu sonuçları retrospektif olarak değerlendirilmiştir. Materyel ve Metod: Bu çalışmaya stress inkontinans ve benign jinekolojik proplemi olan 58 hasta alınmıştır. Ortalama takip süresi 36 aydır (18-40). Preoperatif dönemde hastalara pelvik muayene, üretral hipermobilite tayini için Q tip test ve Boney testi, sistoskopi, multichannel water sistometri ,abdominal ultrasonografi yapılmıştır. Abdominal leak point basınç değeri 65 cm H2Odan az olan vakalar intrinsik sfinkter yetmezliği olarak kabul edilmiştir. Tüm vakalara insitu vajinal wall sling ve 42 vakaya anterior onarım ve 12 vakaya abdominal histerektomi yapılmıştır. Sonuçlar: Cerrahi sonuçlar pet testi ve hasta memnuniyetine göre değerlendirildi. Sonuçlar kür, düzelme ve yetersizlik olarak sınıflandırıldı. Hasta memnuniyetine göre %75.8 kür, %13,7 düzelme ve %10.8 başarısızlık olarak tespit edilmiştir, pet testine göre %72.4 kür, %15.5 düzelme ve %12.06 başarısızlık bulunmuştur. Komplikasyon olarak 4 vakada 4 hafta içerisinde düzelen üriner retansiyon gelişmiştir. Yorum: Stress inkontinansla beraber bulunan beningn jinekolojik problemlerin cerrahi tedavisi sırasında insitu vajinal wall sling uygulanmasının güvenle yapılabileceği belirlenmiştir.
ABSTRACT
Purpose: We investigated the results of insitu vaginal wall sling procedure formal cystocele repair and abdominal hysterectomy in patients with leiomyoma uteri. Material and Methods: A total of 58 patiens with stress incontinence and benign gynecological problems were included in this study. Mean and median follow up were 36 months (range 18 to 40). A pelvic examination to asses urethral hypermobility with Q tip and Boney tests, multi channel water cystometry and pelvic ultrasonografy were included in the preoperative evaluation. Abdominal leak point pressure values of less than 65 cm H2O were accepted as intrinsic sphincter deficiency. We performed insitu vaginal wall sling operations in 58 patients, of whom 42 undergone colporaphy anterior and 12 abdominal hysterectomy at the same session. Results: Results of surgery were clasified according to the pad test and patient questionnaire. Cure, improvement and failure rates were 75.8%, 13.7%, 10.8% respectively for 36 months follow up according to questionnare analysis. These rates were 72.4 %, 15.5 %, 12.06 % according to the results of pad tests. Four paients had temporary urinary retantion problem which resolved in four weeks periods. Conclusions: Insitu vaginal wall sling procedure could be combined with surgical treatment of bening gynecologic problems at the same session.
