ÖZET
Amaç: Pelvik ultrasonografi sırasında endometrium kalınlığı artmış olarak izlenen asemptomatik postmenopozal kadınların klinik yönetimi konusunda kabul görmüş standart bir yaklaşım bulunmamaktadır. Bu prospektif çalışmada endometrium kalınlığı >5 mm olan asemptomatik postmenopozal kadınlardaki endometrial örnekleme ile ilişkili ilk sonuçlarımız bildirilmektedir. Gereç ve Yöntemler: Fakültemiz jinekoloji polikliniğine postmenopozal kanama dışında başka nedenlerden dolayı başvurmuş ve geçmişte hormon replasman tedavisi almamış olan ve halen de almayan ve transvaginal ultrasonografi ile endometrium kalınlığı >5 mm olan toplam 78 postmenopozal hasta çalışmaya dahil edildi. Hastaların demografik özellikleri yanı sıra tıbbi özgeçmişleri (diyabet ve hipertansiyon), sigara kullanımı, parite, toplam emzirme süresi ve ölçülen endometrium kalınlığı kaydedildi. Endometrial örnekleme kürtaj veya pipelle ile ultrasonografik ölçümden itibaren 3 gün içerisinde yapıldı. Hastaların üçü, endometrial örneklemeyi kabul etmediklerinden (n= 2) veya müdahaleye engel teşkil eden ortopedik sorunlardan dolayı (n= 1) değerlendirme dışında bırakıldı. Bulgular: Toplam 78 hastadan endometrial örneklemeyi kabul eden 75'inin yaş ortalaması, menopoz süresi ve vücut kütle indeksi (VKİ) sırasıyla 52.1 ± 5.7 yıl, 5.1 ± 4.4 yıl ve 29.0 ± 5.7 kg/m2 idi. Diyabetik, hipertansif ve sigara içenlerin sayısı sırasıyla 9 (%12), 23 (%30.7) ve 7 (%9.3) idi. Olguların 38'inde (%50.6) atrofik endometrium, 8'inde (%10.7) normal endometrium, 16'sında (%21.3) proliferatif/sekretuar endometrium, 2'inde (%2.7) atipisiz endometrial hiperplazi, 2'sinde (%2.7) atipili endometrial hiperplazi ve 9'unda (%12) endometrial polip saptandı. Polip saptanan olguların 3'ünde beraberinde atipisiz endometrial hiperplazi de mevcuttu. Hiçbir hastada endometrial kanser saptanmadı. Patolojik endometrial histoloji bulunan olguların (n= 13) endometrium kalınlığı (15.3 ± 8 mm ve 6.9 ± 1.7 mm; p< 0.001) ve VKİ'leri (32.2 ± 5.6 mm ve 28.2 ± 5.5 mm; p= 0.02) selim histoloji saptananlardakinden (n= 62) büyüktü. Menopoz süresi, parite, emzirme süresi ve sigara kullanımı bakımından her iki grup arasında anlamlı fark yoktu (p> 0.05). Sonuç: Vaka sayısı az olmakla birlikte, endometrium kalınlığı >5 mm olan asemptomatik postmenopozal kadınlarda yüksek oranda patolojik endometrial bulgulara rastlanıldı. Bu nedenle bu hastalarda endometrial örnekleme önerilebilir.
ABSTRACT
Objective: The management of asymptomatic postmenopausal women with a thickened endometrium is controversial. This prospective observational study provides the histopathological findings of endometrial samplings in asymptomatic women with an endometrial thickness > 5 mm. Material and Methods: A total of 78 postmenopausal women who admitted to our gynecological department for complaints other than postmenopausal bleeding, who were currently not receiving or had received hormone replacement therapy and have had her last menstrual period > 1 year ago, were included in this study. Demographic features of the patients as well as their past medical history (hypertension, diabetes mellitus), smoking habitus, parity, the total duration of breast feeding and endometrial thickness measured by tranvaginal ultrasound were recorded. Endometrial sampling, performed by dilatation and curretage or pipelle, was carried out within three days after the ultrasonographic measurement. Three patients were excluded from the final analysis as they gave either no consent (n= 2) or their physical condition precluded the surgical procedure (n= 1). Results: The age, time since menopause body mass index (BMI) of the 75 patients were 52.1 ± 5.7 years, 5.1 ± 4.4 years and 29.0 ± 5.7 kg/m2, respectively. Diabetes mellitus, hypertension, and smoking were present in 9 (12%), 23 (30.7%) and 7 (9.3%) women, respectively. An atrophic endometrium was present in 38 (50.6%), a normal endometrium in 8 (10.7%), a proliferative/secretuary endometrium in 16 (21.3%), an endometrial hyperplasia without atypia in 2 (2.7%), an endometrial hyperplasia with atypia in 2 (2.7%) and an endometrial polyp in 9 (12%) patients. Three patients with an endometrial polyp also had endometrial hyperplasia without atypia. Endometrial typerplasia was not present in any case. The endometrial thickness (15.3 ± 8 vs. 6.9 ± 1.7 mm; p< 0.001) and BMI (32.2 ± 5.6 vs 28.2 ± 5.5 kg/m2; p= 0.02) in patients with pathological endometrial findings (n= 13) were was greater than those with benign endometrial findings (n= 62). The duration since menopause, parity, total duration of breast-feeding and smoking habitus between both groups were not comparable (p> 0.05). Conclusion: Although the sample size of the study is small, asymptomatic postmenopausal women with an endometrial thickness >5 mm have a high prevalence of endometrial pathologies. Therefore, endometrial biopsi may be considered in such patients.
