The Evaluation of the Levels of Vascular Endothelial Growth Factor and Soluble FMS Like Tirosine Kinase-1 in Preeclamptic and Normotensive Pregnant Women

ÖZET
Giriş: Preeklampside trofoblastların uterusun spiral arteriyollerine yeterince invazyon yapmaması, damarlarda düşük kapasite ve yüksek dirence yol açmaktadır. Vasküler endotelyal büyüme faktörü (VEGF) hipoksi ile tetiklenen anjiyogenezisten sorumlu bir moleküldür. Solubl FMS benzeri tirozin kinaz-1 (sFlt-1) ise VEGF etkisini antagonize eder. Gereç ve Yöntemler: Çukurova Üniversitesi Tıp Fakültesi, Kadın Hastalıkları ve Doğum Bölümüne başvuran normotansif ve preeklamptik gebelerden alınan kan örneklerinde serbest VEGF ve sFlt-1 düzeyleri incelendi. Serum örnekleri ELISA yöntemiyle değerlendirildi. Bulgular: Çalışma ve kontrol gruplarında 60 hasta vardı. Preeklamptik gebelerde kontrol grubu ile karşılaştırıldığında plazma serbest VEGF düzeyleri belirgin olarak düşük (4-11 pg/mL'ye karşılık 9-24 pg/mL; p= 0.002) iken plazma sFlt-1 düzeyleri ise belirgin olarak yüksekti (4899-9962 pg/mL'ye karşılık 900-3954 pg/mL; p= 0.002). Sistolik ve diyastolik kan basıncı ile plazma sFlt-1 düzeyi arasında pozitif bir ilişki, plazma serbest VEGF düzeyi ile sistolik ve diyastolik kan basıncı arasında negatif bir ilişki mevcuttu. Sonuç: VEGF yeni damar oluşumunu tetikleyen, endotel hücrelerine özgü bir büyüme faktörüdür. sFlt-1 ise VEGF reseptörüne bağlanarak onu etkisiz hale getiren bir proteindir. Deney hayvanlarına sFlt-1 verilmesiyle preeklampsi benzeri bir klinik tablo ve böbreklerde glomerüler endoteliyozis oluşturulmuştur. Yaptığımız çalışmada, preeklamptik hastalarda plazma serbest VEGF düzeyleri azalma, sFlt-1 düzeyleri ise artma göstermektedir. Bu bulgular preeklampsi patogenezinde anjiyogenezisin yetersizliğine bağlı hipoksi gelişiminde sFlt-1'in etkili bir rolü olduğunu düşündürmektedir.
ABSTRACT
Objective: The incomplete invasion of the trophoblasts in the spiral arteries is the cause of the low vascular capacity and increased resistance in preeclampsia. Vascular endothelial growth factor (VEGF) is a molecule which is responsible of the hypoxia triggered angiogenesis. On the other hand; soluble fms-like tirosine kinase-1 (sFlt-1) inhibits the effect of the VEGF. Material and Methods: In this study serum free VEGF and sFlt-1 levels of the preeclamptic and normotensive women who admitted to the Obstetric and Gynecology Unit Faculty of Medicine of the Cukurova University are examined. The samples are measured by an enzyme-linked immunosorbent assay (ELISA). Results: The study and control group contained 60 patients. Preeclamptic women had significantly low levels of free VEGF comparing to the normotensive women (4-11 pg/mL versus 9-24 pg/mL; p= 0.002); while sFlt-1 levels were considerably increased (4899-9962 pg/mL versus 900-3954 pg/mL; p= 0.002). Systolic and diastolic blood pressure had a positive correlation with the sFlt-1 levels and negative association with the VEGF. Conclusion: VEGF is an endotel specific factor which stimulates the production of the new vessels. On the other hand sFlt-1 binds to the VEGF receptor and inhibits its effect. The clinical features of the preeclampsia and renal glomerular endotheliosis were observed when sFlt-1 is given to the animals. Our study showed a significant decrease on the levels of the free VEGF and increase on the levels of the sFlt-1. We thought that sFlt-1 had an important role in the hypoxia due to the angiogenetic insufficiency.
