ÖZET
Amaç: Normal gebelerde serum AFP seviyelerinin tespit edilerek, gebelik haftasına göre grafiğinin çizilmesi ve NTD'i olan olgularda maternal serum AFP değerlerinin ortalama ne kadar üzerine çıktığının tesbiti ve de testin güvenirliğinin araştırılması. Çalışmanın Yapıldığı Yer: Ege Üniversitesi Tıp Fakültesi, Kadın Hastalıkları ve Doğum AD, İzmir. Gereç ve Yöntemler: Ege Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Polikliniğine müracaat eden 351 gebede MSAFP düzeyleri 16. gebelik haftasından sonra tespit edildi. Ölçümler Delfia hAFP kit'i kullanılarak enzim immunassay yöntemi ile yapıldı. Verilerin istatistiksel değerlendirilmesinde, varyans ve regresyon analizi uygulandı. Bulgular: MSAFP seviyesi araştırılan 351 olguda en düşük değerin 16. haftada (17 ng/ml), en yüksek değerinde 33. haftada (171 ng/ml) olduğu saptandı. Olguların 14'ünde Ultrasonografik tetkikde anomali saptandı ve bu olgularda MSAFP düzeyinin normalin en az 3 misli kadar yüksek olduğu görüldü. Bir olguda NTD olmasına rağmen MSAFP seviyesi normal iken, bir olguda da MSAFP düzeyi yüksek bulunmasına rağmen anomali mevcut değildi. Sonuç: Gebede MSAFP ölçümü antenatal izlemde rutin tetkikler arasına girmelidir. MSAFP değerinin gebelik haftasına göre normal ortalamanın 3 mislinden yüksek bulunması halinde ultrasonografik tetkik yapılarak NTD'nin araştırılması uygun olacaktır. Bu testin güvenilirliği oldukça yüksek bulunmuştur.
ABSTRACT
Objective: The mean values of maternal serum alpha fetoprotein in healthy pregnant women according to the gestational weeeks of pregnancy were investigated and the increasement of the MSAFP level at the neural tube defect cases were determined. The reliability of the test was established. Institutuon: Aegean University, Medical Faculty Obstetrics and Gynecology Department, İzmir. Meterial and Methods: MSAFP values of 351 pregnant women after the 16th week of gestation who referred to the Aegean University Medical Faculty Obstetrics and Gynecology Department were determined. Delfia kit was used. Data were statistically analyzed by the variance and regression analysis. Findings: In healthy pregnant women, lowest MSAFP value was at the 16th week of gestation (17 ng/ml), where as the highest value was at the 33th week of gestation. NTDs were found ultrasonographically in 14 cases who have at the least 3 fold increasement at the MSAFP mean livels. In one case false positive and in another case false negative results were present. Results: MSAFP analysis should be done routinly at the antenatal controls of the pregnant women. Patients who have three fold increasement of MSAFP mean values should be ultrasonographically examined in respect of NTD. The reliability of the test was found to be sufficient.
