ÖZET
Amaç: Preterm prematür membran rüptürü (PPROM) tanısıyla konservatif tedavi planlanan gebelerde feto-maternal enfeksiyonun erken tahmininde ucuz ve noninvazif bir yöntem olarak seri nonstress test (NST) takibinin güvenilirliğinin incelenmesi. Materyel ve Metod: PPROM tanısıyla Kasım 1997- Aralık 1998 tarihleri arasında Dr. Zekai Tahir Burak Kadın Hastanesi Yüksek Riskli Gebelikler servisine yatırılan, eylemde olmayan ve yatışında klinik ve laboratuvar olarak enfeksiyon bulgusu taşımayıp konservatif tedavi planlanan 26-35 gestasyon haftasındaki 105 gebe doğuma kadar enfeksiyon yönünden klinik semptomlar ve laboratuvar bulgular ile, fetal iyilik hali ise günlük fetal hareket ve NSTlerle takip edildi. Her hastaya ilk klinik değerlendirmede kültürleri alındıktan sonra antibiyotik profilaksisi, fetal akciğer matüritesini hızlandırmak için kortikosteroid ve gerekli görüldüğünde tokoliz uygulandı. Bu hastalar içerisinde hastanedeki izlemi sırasında kültürlerle de kanıtlanmış enfeksiyon tespit edilen 27 hasta ve doğuma kadar herhangi bir enfeksiyon tanısı almayan 78 hastanın NST takipleri, 1. dakika ve 5. dakika apgar skorları, latensi periodları, doğumdaki gestasyon yaşları, doğum ağırlıkları, neonatal komplikasyonlar ve perinatal mortalite oranları karşılaştırıldı. Bulgular: Enfeksiyon gelişen hastalarda izlem sırasında NSTlerin reaktivitesindeki bozulmanın enfeksiyon gelişmeyen hasta grubundakinden anlamlı olarak fazla olduğu (p= 0.001), benzer şekilde enfeksiyonlu grupta7nin altında 1. ve 5. dakika apgar skoru, neonatal komplikasyonlar (sepsis,respiratuar distres sendromu (RDS), nekrotizan enterokolit (NEK), pnömoni) ve perinatal mortalite oranlarının da yüksek olduğu görüldü (p< 0.001). Sonuç: PPROM tanısıyla konservatif tedavi planlanan hastalarda seri NST takipleriyle izlemde, NSTnin aktivitesinde bozulmanın gözlenmesinin feto-maternal enfeksiyonun tahmininde ucuz, noninvazif ve güvenilir bir yöntem olduğu kanısındayız.
ABSTRACT
Objective: To determine the reliability of serial nonstress tests (NST) as a noninvasive and cost-effective method for the early detection of feto-maternal infection in patients with preterm premature rupture of membranes (PPROM). Material and Method: Between November 1997-December 1998, 105 pregnant women of 26-35 gestational week with the diagnosis of PPROM, hospitalized in High Risk Pregnancy ward of Dr. Zekai Tahir Burak Womens Hospital were included into the study, proven that neither of them were in labour nor showing any signs or symptoms of infection at the time of admission. They were given expectant management by observing clinical and laboratory signs and symptoms of infection as well as daily fetal movements and NST for the determination of fetal well-being until delivery. Clinical examination and cultures were done for each patient on admission, then antibiotic prophylaxis, corticosteroids for the stimulation of fetal lung maturity and tocolysis when needed were applied. During the follow-up 27 of the patients developed maternal and/or fetal infections proven by cultures and remaining 78 patients reached delivery without any infections. Between the two groups serial NSTs , latency periods, gestational age at delivery, birth weights, 1. and 5. minute apgar scores, neonatal complications and perinatal mortality rates were compared. Results: It was observed that in the infection group deterioration of the reactivity of NST during the follow-up as well as 1. and 5. minute apgar scores of <7, neonatal complications (sepsis, pneumonia, RDS, NEC) and perinatal mortalitry rates were higher than those in the non-infection group and the differences were found to be statiscally significant (p<0.001).
