The Comparison Of The Effects On Kupperman Indices, Hot Flushes And Changes Of Endometrial Thickness At Transvaginal Sonography After The Therapy Of Tibolone, Estradiol Hemihydrate-neta And Conjugated Estrogene-MPA Combinations

ÖZET
Amaç: Bu çalışmanın amacı postmenopozal hormon replasman terapisi olarak tibolon, östradiol hemihidrat-NETA ve konjuge östrojen-MPA kullanan kadınlarda transvajinal sonografi ile ölçülen endo-metrium kalınlıklarındaki değişikliklerin ve söz konusu hormono-terapilerin Kupperman indeksinde oluşturduğu değişikliklerin ve sıcak basması sıklığı üzerine olan etkilerinin saptanmasıdır. Gereç ve Yöntemler: Çalışmaya Ege Üniversitesi Menopoz Polikliniği'ne Ocak 2002- Ocak 2004 tarihleri arasında başvuran ve doğal menopozda olan hastalar arasından seçilen ve çalışma kapsamındaki hormonal rejimi kullanmakta olan toplam 164 olgu dahil edildi. Çalışma retrospektif olarak, olguların poliklinik kartları taranarak ve klinik bilgilerine ulaşılarak gerçekleştirildi. Tüm olguların bazal (0. ay), 1., 3. ve 6. aylarda ölçülen endometrial kalınlıkları ve klimakterik yakınmalarındaki değişiklikler değerlendirildi. Çalışmaya dahil edilen hormon replasman tedavisi preparatları; Tibolon (Livial 2,5 mg, Organon), östradiol hemihidrat-noretisteron asetat (NETA) kombinasyonu (Activelle tablet, östradiol hemihidrat 1 mg ve noretisteron asetat 0.5 mg, Novo Nordisk) ve konjuge östrojen-medroksiprogesteron asetat kombinasyou (Premelle 2.5 draje, konjuge östrojen 0.625 mg ve medroksiprogesteron asetat (MPA) 2.5 mg, Wyeth) preparatları idi. Toplam 164 olgunun 70'i Tibolon, 34'ü östradiol hemihidrat-NETA ve 60 olgu konjuge östrojen-MPA kullanmakta idi. Olguların endometrium kalınlıkları transvajinal sonografi ile ölçüldü. Bulgular: Hormon replasman tedavisi şekline göre sıcak basmaları sıklıkları arasındaki farka bakıldığında, bazal değerler ve üçüncü ay sonundaki değerler arasında anlamlı farklılık saptanmazken, altıncı ayın sonunda Tibolon ve konjuge östrojen-MPA grubunda, östradiol hemihidrat-NETA grubuna göre anlamlı olarak daha az sıcak basması atağına rastlandı (p= 0.02). Her üç tedavi grubunun Kupperman indeksine olan etkileri karşılaştırıldığında her üç grup arasında anlamlı farklılık saptanmadı (p> 0.05). Söz konusu tedavi şekilleri ile takip sırasında endometrial kalınlıkta meydana gelen değişiklikler incelendiğinde, yine her üç grup arasında anlamlı endometrial kalınlık farkı saptanmadı (p >0.05). Sonuç: Tibolon, klimakterik yakınmaları gidermede konvansiyonel kombine hormon replasman tedavisi preparatları gibi etkin bir tedavi seçeneği olmasının yanı sıra, altı aylık takipte endometrial kalınlıkta anlamlı artış oluşturmamaktadır.
ABSTRACT
Objective: To compare the changes of endometrial thickness at transvaginal sonography, frequency of hot flushes and the Kupperman indices after the therapy of tibolone, estradiol hemihydrate-NETA and conjugated estrogene-MPA combinations. Material and Methods: Onehundred and sixty four patients who have physiological menopause and taking hormonotherapy were selected from the medical records of Menopause Clinic of Ege University Medical Faculty Gynecology Department between January 2002-January 2004 for this retrospective study. The beginning, first, third and sixth months of endometrial thicknesses and the changes in the climacteric complaints of the patients are evaluated. The HRT drugs which were used in this study were: Tibolone (Livial 2.5 mg, Organon), the combination of estradiol hemihydrate and norethisteron acetate (NETA) (Activelle tablet, estradiol hemihydrate 1 mg and norethisteron acetate 0.5 mg, Novo Nordisc) and conjugated estrogene-medroxyprogesterone acetate combination (Premelle 2.5 pills, cojugated estrogene 0.625 mg and medroxyprogesterone acetate (MPA) 2.5 mg, Wyeth). The distribution of the groups were as follows: 70 out of 164 cases were using Tibolone, 34 cases were using Estradiol hemihydrate-NETA and 60 cases were using conjugated estrogene-MPA in the study. Endometrial thickness of the patients were measured by transvaginal sonography. Results: There was no significant difference between the HRT groups in the beginning of therapy and after three months with regard to frequency of hot flushes. However, at the end of six months the frequency of hot flushes was significantly low in tibolone and conjugated estrogene-MPA groups than estradiol hemihydrate-NETA group (p= 0.02). The effect on Kupperman index were not different between the groups (p> 0.05). There was also no significant difference between the endometrial thickness measurements of the groups (p> 0.05). Conclusion: Tibolone is an effective therapeutic choice in the treatment of climacteric symptoms when compared with the conventional combined HRT drugs and has no significant effect on endometrial thickness.
