ÖZET
[Twin Reversed Arterial Perfusion (TRAP)]-İkizde Ters Dönmüş Arteriyel Perfüzyon sekansı, sadece monokoryonik monozigotik ikiz gebeliklerde görülen kötü prognozlu bir sendromdur. Otuz beş bin doğumda, 100 monozigot ikizde 1 görülme insidansı mevcuttur. TRAP sekansında multipl anomalilere sahip, kalbi gelişmemiş "nonviable" bir fetus ile bu fetusu plasentadaki vasküler anastomozlar yoluyla besleyen pompa fetusun varlığı söz konusudur. Multipl anomalili ikizde gövde ve ekstremite hareketlerinin varlığına rağmen kalbin ultrasonografi (USG) ile gösterilememesi ve plasentadaki vasküler anastomozların Doppler USG ile ortaya konması ile antenatal dönemde tanı konabilir. Akardiyak ikiz için mortalite %100 iken, pompa ikizin mortalitesi %50 civarındadır. Mortalite kalp yetmezliğine bazen de polihidramniosun neden olduğu erken doğum ve prematüriteye bağlı olabilir Antenatal takipleri farklı merkezlerde yapılan 34. gebelik haftasında tanısı konan TRAP sekansı olgusunu sunarak, bu olgulardaki tanı yöntemleri, klinik prezentasyonlar ve yönetim seçeneklerini tartışmayı amaçladık.
ABSTRACT
Twin Reversed Arterial Perfusion (TRAP) sequence is a syndrome, seen in just monochorionic monozygotic twin pregnancies with poor prognosis. Incidence is one in 35.000 births and one in 100 monozygotic twin pregnancies. It is characterized with a non-viable recipient fetus exhibiting lethal anomalies including acardia and a pump fetus supplying blood by vascular communications in the placenta. It could be diagnosed in antenatal period despite movement of body and extremities with no detection of heart by ultrasound of twin with multiple anomalies and detection of vascular anastomosis in the placenta by Doppler USG. Mortality rate of acardiac twin is 100%. Also pump twin has the mortality rate of 50%. Mortality is usually due to heart failure or premature labor caused by polihydramnios of pump fetus. We present a TRAP sequence case with 34 weeks' of gestation followed by different centers and diagnosed in late antenatal period. Diagnose methods, clinical presentation and management options were discussed in this cases.
