Umblical Cord Leptin Levels In Macrosomic And Low-birth Weight Infants

ÖZET
Amaç: Bu çalışmada amacımız, umblikal kord leptin düzeylerinin doğum ağırlığı ile ilişkisini saptamak, fetal ve neonatal hayatta, nutrisyonel durum ile leptin düzeylerinin ilişkisini araştırmaktır. Yöntemler: Bu çalışmaya kliniğimize başvuran 44 olgu alınmıştır. Çalışmaya sistemik hastalığı olan (DM, hipertansif hastalık, konnektif doku hastalığı, kardiak hastalık, böbrek yetmezliği vb.) gebelerin infantları ve 37. gestasyonel haftadan önce doğan infantlar dahil edilmemiştir.Sonuçların istatistiksel değerlendirmesinde SPSS 9.0 istatistik programı, t-testi (2-independent t-test) ve varyans analizi (ANOVA) testleri kullanılmıştır. Bulgular: Umblikal kord leptin düzeyleri ile doğum ağırlıkları arasında pozitif korelasyon saptanmıştır (r=0,967, p<0,0001).Düşük doğum ağırlıklı bebekler ile normal bebekler arasında, umblikal kord leptin düzeyleri bakımından anlamlı bir farklılık bulunmuştur (p<0.0001). Normal doğum ağırlıklı bebekler ile makrozomik bebekler arasında da, umblikal kord leptin düzeyleri bakımından anlamlı bir farklılık vardır (p<0.0001). Sonuçlar: Çalışmamız sonuçlarına göre, plazma leptin seviyeleri makrozomik bebeklerde artmakta, düşük doğum ağırlıklı bebeklerde azalmaktadır. Düzeyleri yağ dokusu hacmi ile direkt orantılı olarak çıkmıştır. Leptinin, bebek doğum ağırlığını belirleyen bağımsız bir faktör olduğu ve fetal büyüme anormalliklerinde, antenatal takip için kullanılabilecek değerli bir metabolik hormonal parametre olduğu inancındayız.
ABSTRACT
Objective: Our objective was to investigate the relationship between umblical cord leptin levels and birthweight, to determine the impact of leptin levels on the nutritional status in fetal and neonatal life. Methods: 44 patients who admitted to our clinic were included in this study. The infants of the pregnants who had systemic diseases (such as diabetes mellitus, hypertensive disorders, connective tissue disease, cardiac disease, renal failure etc.) and the infants who had born before 37th gestational week were excluded. SPSS 9.0 statistical program, t-test (2-independent t test) and variance analysis test (ANOVA) were used for the statistical analysis of the results. Results: A positive corelation was found between the umblical cord leptin levels and the birthweights of the infants (r=0,967, p<0,0001). There was a statistically significant difference between the umblical cord leptin levels of the low birthweight infants and normal birthweight infants (p<0,0001). And also a significant difference was found between umblical cord leptin levels of normal birthweight infants and that of macrosomic infants (p<0,0001). Conclusions: According to our studys results, plasma leptin levels increase in macrosomic infants and decrease in low birthweight infants. The levels of leptin are directly proportional to the amount of the adipose tissue mass. We believe that leptin is an independent factor for determining the birthweight of the infants and a valuable metabolic hormonal parameter that can be used for antenatal follow-up of the fetal growth abnormalities.
