Value Of Increased Umblical Artery S/D Ratio To Predict Intrapartum And Postpartum Fetal Outcome

ÖZET
Amaç: Gebeliğin son beş günü içinde yapılan umblikal arter S/D ölçümünün intrapartum ve postpartum fetal prognozu belirlemedeki önemini saptamak. Gereç ve Yöntemler: Çalışmaya Ocak 2000-Kasım 2002 tarihleri arasında umbilikal arterlerinde yüksek S/D oranı saptanan 62 hasta ve kontrol grubu olarak umbilikal arter S/D oranı normal olan 156 hasta alındı. Bu gruplar antepartum risk faktörleri; preeklampsi, kronik hipertansiyon, gestasyonel diyabet, intrauterin gelişme kısıtlılığı, oligohidramniyos, non reaktif NST, tatminkar olmayan fetal kalp atımı trasesi nedeniyle sezaryen oranı, doğum ağırlığı, 1. ve 5. dakika Apgar skorları, umbilikal arter kan gazları, yenidoğan yoğun bakım ünitesine (YBÜ) yatış yönünden karşılaştırıldı. Bulgular: Umblikal arter S/D oranı normal olanlar ile yüksek olanların ortalama anne yaşı, gravidaları, son menstrüasyon tarihine göre gebelik haftaları benzer bulundu. Tatminkar olmayan fetal kalp atımı trasesi nedeniyle sezaryen doğum oranı S/D yüksek grupta %48 (n=30) iken S/D normal grupta %10 (n=17) idi (p<0.001). Olguların eksternal fetal monitörizasyon sonuçları değerlendirildiğinde non reaktif NST, variabilite kaybı, variable deselerasyon ve geç deselerasyon oranlarının S/D yüksek grupta S/D normal gruba göre anlamlı olarak fazla olduğu bulundu. S/D yüksek grupta düşük 1. ve 5. dakika Apgar skoru, fetal asidoz ve yenidoğan yoğun bakım ünitesine yatırılma oranının fazla olduğu bulundu. Fetal asidozu tahmin etmedeki en önemli risk faktörlerinin gebeliğe bağlı hipertansiyon varlığı (OR=1.6, %95 CI=1.05-2.7, p=0.03) ve non-stress test'de variabilite kaybı olduğu (OR=2.6, %95 CI=1.2-5.9, p=0.01) bulundu. Sonuç: Gebeliğin son beş günü içinde umblikal arter Doppler ölçümünde bulunan artmış S/D oranı fetal iyilik halini değerlendirmede ancak gebeliğe bağlı hipertansiyon ve non-stress test'de variabilite kaybı olduğunda önemli olabilecek bir bulgudur.
ABSTRACT
Objective: To identify the predictive value of Doppler evaluation of the umblical artery S/D ratio performed on the last five days of gestation on intrapartum and postpartum fetal prognosis. Material and Methods: Data were collected on 62 pregnancies whose umblical artery Doppler evaluations revealed elevated S/D ratios, and on 156 pregnancies as a control group whose umblical artery Doppler evaluations revealed normal S/D ratios from January 2000 to November 2000. These two groups were compared according to the antepartum risk factors; preeclampsia, chronic hypertension, gestational diabetes mellitus, intrauterine growth retardation, oligohydramnios, non-reactive NST, cesarean delivery for non-reassuring fetal heart rate pattern, neonatal birth weight, 1-minute and 5-minute Apgar scores, umblical artery blood gas analyses, and admission to the intensive care unit. Results: There was no significant difference in the mean maternal age, gravida, and gestational age according to last menstrual period between patients whose umblical artery S/D ratios were normal compared to those elevated. The incidence of cesarean delivery for non-reassuring fetal heart rate pattern between patients with elevated 48% (n= 30) and normal S/D ratios 10% (n= 17) were significantly different (p< 0.001). According to the external fetal heart monitorization, patients with elevated S/D ratio had statistically significant higher rates of non-reactive NST, loss of variability, variable decelerations and late decelerations compared to the group with normal S/D ratio. Patients with elevated S/D ratios were found to have lower 1-minute and 5-minute Apgar scores, fetal acidosis and higher incidence of admission to the neonatal intensive care unit. The most significant risk factors to predict fetal acidosis were the presence of pregnancy induced hypertension (OR= 1.6, 95% CI= 1.05-2.7, p= 0.03) and loss of variability at fetal heart rate tracings (OR= 2.6, %95 CI= 1.2-5.9, p= 0.01). Conclusion: The Doppler evaluation of the umblical artery during last five days of gestation revealing elevated S/D ratios could only predict fetal well-being in the presence of pregnancy induced hypertension and in the presence of loss of variability during fetal heart rate tracings.
